Below you will find all the forms that are necessary to move through the Contextual Education process. All master’s students in programs with ConEd curricular requirements must complete the appropriate forms by the deadlines listed on the Contextual Education Process Checklist.

Meeting Request Form
Students are required to meet with at least one Director of the Contextual Education Program before pursuing a ConEd option. In the meeting, the Director(s) will discuss with the student the various ways they can complete the Contextual Education requirements for their degree program, will discuss site availability and fit with the student’s interest, any ordination or denominational needs, and any other questions or concerns that the student brings to the ConEd process.
Students who have already fulfilled their ConEd curricular requirement can also meet with the Director(s) at any time to dream up additional ConEd projects or placements, or just for support in their vocational discernment, professional development, spiritual formation, or career networking.
Once you complete the form below, someone from the ConEd Office will reach out to schedule your meeting. Meetings can happen in-person or over Zoom, depending on what you prefer and Director availability.

Weekly Time Reporting
This hour tracking form should be completed weekly to account for any on-site hours at your internship placement. Please refer to your Integrating Theology and Practice syllabus, which outlines the start and end date of each week. Total hours logged through this form will be calculated at the end of the semester and used to determine points towards your final grade according to the syllabus.

Placement/Project Request Form
The Placement/Project Request Form should be completed after you have met with one of the Contextual Education Directors and either:
- Have an offer to complete your placement at a site, or
- Have a faculty member who has agreed to supervise your project

The Learning Agreement form should be developed with your on-site supervisor at the beginning of the semester you are completing your placement.
