Forms for students at Boston University School of Public Health
Forms may be scanned and emailed to the SPH Registrar’s Office.
Add Drop Form
- Who Uses: Students adding a class after the semester starts, with instructor approval
- Return to: SPH Registrar’s Office (obtain instructor signature if class has met)
- Deadline: Before second class meeting date
Certificate Declaration or Change Form
- Who Uses: MPH and Dual students who wish to declare, add, change, or drop a certificate
- Return to: Online form, hitting submit returns information to SPH Registrar’s Office.
- Deadline: None except graduation, certificate change forms will not be processed in the week before a new registration period opens
Course Substitution Form
- Who Uses: Students who wish to use an SPH course as a substitute for a degree requirement
- Return to: SPH Registrar’s Office after obtaining required approvals
- Deadline: Should be submitted before student submits a graduation application
Course Audit Request Form
- Who Uses: Students who wish to change the status of a registered course from credit to audit status or vice versa
- Return to: SPH Registrar’s Office after obtaining instructor approval
- Deadline: Needs to be submitted by “Drop Without a W” deadline for the course
Directed Study/Research Form
- Who Uses: Students who will register for a directed study or directed research project
- Return to: SPH Registrar’s Office with faculty signature
- Deadline: May add a 1 or 2 credit directed study up to the 7th week of the semester
Doctoral Student Continuing Study Status Form
- Who Uses: Doctoral students who complete their coursework
- Return to: SPH Registrar’s Office with program director signature
- Deadline: Up to the 5th week of the effective semester
Graduation Application May 2024
- Who Uses: Degree students who will complete all their degree requirements by the end of the Spring 2024 semester
- Return to: SPH Registrar’s Office
- Deadline: November 10, 2023
Graduation Application January 2024
- Who Uses: Degree students who will complete all their degree requirements by the end of the Fall 2023 semester
- Return to: SPH Registrar’s Office
- Deadline: June 10, 2023
Leave of Absence/Withdrawal Form
- Who Uses: Degree students seeking leave of absence or withdrawal
- Return to: SPH Registrar’s Office (some students will need additional signatures)
- Deadline: Before start of semester
Transfer Credit/Waiver Request Form
- Who Uses: Degree students who wish to apply credit for graduate courses taken outside of BUSPH
- Return to: SPH Registrar’s Office with supporting documents
- Deadline: Before the course starts, allow 4 to 6 weeks for processing
Verification Request Form
- Who Uses: Students who need verification of enrollment or graduation (standard verification or filling out a form).
- Return to: SPH Registrar’s Office
- Deadline: Allow 3-5 business days for processing
Waitlist Form Spring 2024
- Who Uses: Degree students at BU unable to register for an SPH class.
- Return to: Online form, hitting submit returns information to SPH Registrar’s Office.
- Deadline: TBD.
Waitlist Form Summer 2023
- Who Uses: Degree students at BU unable to register for an SPH class.
- Return to: Online form, hitting submit returns information to SPH Registrar’s Office.
- Deadline: The waitlist opens on Thursday, February 16, 2023 at 7:00am and closes for Summer 1 classes on Friday, May 12, 2023. It closes for Summer 2 classes on Friday, June 30, 2023.
Waitlist Form Fall 2023
- Who Uses: Degree students at BU unable to register for an SPH class.
- Return to: Online form, hitting submit returns information to SPH Registrar’s Office.
- Deadline: The waitlist opens on Saturday, April 1 at 9:00am and closes for full-term and 1st half classes on Friday, August 25, 2023. It closes for 2nd half classes on Friday, October 13, 2023.
The following forms are not online forms:
- Name Change Forms – available at BU Registrar’s Office at 881 Commonwealth Avenue and at SPH Registrar’s Office. Must bring legal proof of name change when completing form.
- Grade Change Form – available only to faculty, must be picked up from Registrar’s Office
