Recital Forms & Paperwork
Undergrad students:
Performance Majors: Applied Music Examination Form
Theory/Comp Majors: Email submission of Recital Program to the Academic Coordinator, with department chair cc’d for approval.
Grad students:
Please consult the Grad Student Handbook for detailed explanations of recital requirements per selected track. Please see the DMA Policies & Forms tab below for additional resources.
The Applied Music Examination Form must be submitted for the following graduate recitals:
MM Recitals, DMA Solo Recital, DMA Chamber Recital, and DMA Concerto Recital.
The following DMA Recitals require a form other than the Applied Music Examination form:

DMA Policies and Forms
The Graduate Studies Committee
As a graduate student, you may need to contact the Committee formally for two broad purposes: 1) to submit a petition (please see CFA Forms and Procedures), and 2) to file a proposal for a terminal project, e.g., a master’s thesis or doctoral dissertation. All petitions for exceptions to the required curriculum must be submitted to the Director of Graduate Studies prior to taking the courses in question.
Committee meeting dates for the 2025-2026 academic year are as follows: September 25, October 23, November 13, February 12, March 5, April 9
