Terrier Card Replacement Form

Please complete the form below for a Terrier Card replacement if your card has been lost or damaged.

All students receiving a replacement Terrier Card must upload a new photo (one that has not been previously used on your card) following the photo upload procedures and guidelines.

If you lose or damage your Terrier Card, we can make you a replacement card. Please note there is a replacement fee of $40 for each new card.

1. I must carry my Terrier Card at all times and it may not be used by anyone other than myself; it remains the property of Boston
University and must be surrendered to a University official upon demand.
2. Alteration or use of this card for any unauthorized purpose will result in confiscation, financial penalty, and/ or disciplinary action.
3. I will report any loss, damage, theft, or unauthorized use of my Terrier Card to the Terrier Card Office immediately.
4. A lost, damaged, or stolen Terrier Card can be replaced at the Terrier Card Office for a fee of $40 which will automatically be
charged to my student account. Multiple replacements may result in additional fees of up to $50 in each instance.
