You’re in! Next steps for admitted RISE students
Congratulations on being accepted into the RISE program! We are excited to welcome you to campus. On this page you will find a detailed list of all the forms you will need to submit and the online safety training you will need to complete before arriving on campus.
Due 1–2 weeks after receiving your acceptance email:
- Letter of Intent: This form, along with your $1,000 deposit, must be obtained to secure your place in the program.
Due by May 31:
- Parental Acknowledgement, Consent & Release
- Media Release
- FitRec waiver
- Commuter Permission form: This form is required for non-residential students who will be living with family during the program.

- Patent Agreement for Boston University’s Charles River campus
- Laboratory Student Consent form
- Novartis CELL Permission form
- Internship students only: These forms may or may-not apply to you please review your acceptance email to know which ones you will need.

- Environmental Health & Safety laboratory waiver: This is specific to each laboratory and is attached to your acceptance email.

- Work Permit: This document is only for students working at MIT, Harvard, or Brigham & Women’s Hospital
- Practicum students only
- Medical Forms and requirements
- You may be required to have additional blood tests completed based upon the laboratory you will be working in. If so, these requirements will be listed in your acceptance email:
- TB Test: You will either need two skin tests within a year of each other OR you will need a TB Quantiferon Gold blood test within a year of August 11. You must submit the laboratory result before arriving on campus.
- Hepatitis B titer: This blood test will be necessary regardless of your immunization history and must be completed before arriving on campus. You must submit the laboratory result.
- All students must have two MMR vaccinations.
- Immunization and Physical Form: This gives students permission to be treated by Student Health Services without a parent or guardian present and is required regardless of whether the student is residential or commuter
- ROHP Permission form: This gives students permission to be treated by ROHP, this is required for all students who will be working on either Boston University’s Charles River Campus or the Medical Campus.
- Immunization Record: As you will be working in a laboratory, we require a complete immunization record and you must be compliant with the laws of Massachusetts.
- Meningococcal Waiver and Tuberculosis Record: If a TB test if that is needed for your laboratory, you cannot use the waiver.
- Health History Form: Please also list any dietary restrictions on this form. This will allow us to contact the nutritionist on campus to make sure you have safe food options in the dining hall.
- Medical Insurance: We require either a copy of your medical insurance card or you must purchase BU health insurance.
- You may be required to have additional blood tests completed based upon the laboratory you will be working in. If so, these requirements will be listed in your acceptance email:
Due by June 15:
- Laboratory safety training: Once you receive your BU ID number and create your BU login account (this will be sent by email only to the student from summerhs@bu.edu), you will receive an email with instructions about the mandatory safety training. Without this training you will not be permitted to enter the laboratory.

- Biosafety Level 1 & 2 Training
- Additional trainings that may be asked of you to complete depending on your laboratory in BioRaft:
- Laser Safety
- Bloodborne Pathogen
- Recombinant DNA/IBC
- IACUC Orientation
- BU Animal Science Center New Research Orientation (NRO)
- Additional trainings that may be asked of you to complete on another site. For these trainings you will need to create an account using your BU email and select researcher. After you complete these trainings before June 2, you must email your certificate to rise@bu.edu.
