SOM Ensemble Sick/Emergency Call Out Form

This form is only for short term absences due to illness or emergency. Please fill out this form if you are sick or are experiencing an emergency and can not attend rehearsal today or tomorrow. The ensembles manager may follow up with you regarding your absence on a case-by-case basis. To request approval for an upcoming absence, please fill out the absence request form. If you need accommodations due to an injury, please email the ensembles manager (khurd@bu.edu).
