INSTRUCTIONS: This form is to evaluate the performance of a student who has completed or is nearing completion of an internship or co-op at your organization as part of the College of Engineering's Experiential Education course. The immediate supervisor (or Internship/Co-op Coordinator) should evaluate the student objectively, comparing him/her with other students of comparable academic level, with other personnel assigned to the same of similarly classified jobs, or with individual standards.
