CFA Tuition Exception Form

Eligibility Guidelines:

  • Requests will be considered for documented extenuating circumstances only.
  • Requests submitted more than 90 days past the term in which the exception is being requested may not be considered.
  • Online students should contact Shannon Streets to obtain the Distance Education Activity Report for the semester they are requesting an exception. This should be uploaded with any other supporting documentation.

To request a refund or a late penalty waiver you must:

  • Complete the request form.
  • Provide supporting documentation for each circumstance stated below. All supporting documentation must be signed.
    • Examples of supporting documentation include, but are not limited to:
      • Physician statement (must be on physician’s letterhead)
      • Accident report
      • Written statement from each instructor identifying whether you attended, participated, or contacted them during the term in question

Incomplete requests will not be reviewed. Incomplete appeals are those that do not have all steps completed or are submitted without appropriate documentation.

All petitions will be reviewed by the College of Fine Arts Associate Dean for Academic Programs and Enrollment, for recommendation to the Vice President of Enrollment.

Students will be notified by email of the results of their appeal.
