Transcripts & Institution Evaluations

Transcripts

Transcripts contain all academic information about a student, including classes taken, grades earned, class rank (pre-doctoral students only), GPA (academic year and cumulative), and degree(s) awarded. Each transcript bears the signature of the Henry M. Goldman School of Dental Medicine (GSDM) Registrar and the Boston University seal. Official transcripts are issued in a sealed envelope.

Requests are normally processed within three business days.

How to Request a Transcript

Current Students

Alumni & Former Students
  • Current students must request transcripts online via the GSDM Portal (Kerberos login required).
    • If you are off Campus: log-on to the VPN before going to the GSDM Portal
  • Alumni must submit requests in writing by submitting the linked Transcript Request Form. A secure digital signature is required. You will be asked to validate your email address.

Transcript Delivery Options

Official GSDM Transcripts are only available in paper format at this time. Transcripts are available via:

  • On-campus pick-up (no cost to student/alumni)
    • By appointment. At the time of pick-up, a valid photo ID is required.
  • Standard USPS mail (no cost to student/alumni).
  • FedEx via eSHIP Global (shipping fees will apply).
    • Sign up for an account and log in.
    • Click “Receive documents from Universities,” search Boston University, and select Boston University Dental School.
    • Enter your FedEx delivery address (no PO Boxes; overnight to Canada not available). Note that the system defaults to your account address. You must change it if the document is not being shipped to you.
    • Choose a FedEx service, complete payment, and submit.
    • Once submitted, you will receive confirmation and tracking information by email.

Institution Evaluation Form

Students and alumni may request a letter of recommendation from the Dean of GSDM (i.e., an Institution Evaluation Form) for residency applications through the PASS or for non-PASS purposes as well.

Students and alumni may also request letters of recommendation from the Associate Dean of Students, including personal letters of recommendation and the Professional Evaluation Form via the Postdoctoral Application Support Service (PASS). For more information on those letters, please  click here.
