Letter of Recommendation – Institution

Students may request an Institution Evaluation Form [IEF] from the dean of the Henry M. Goldman School of Dental Medicine using the form below. IEF’s are required for residency applications through the Postdoctoral Application Support Service (PASS) but may also be required for non-PASS programs.

The form will be submitted electronically, and the information contained within will be used by the Office of the Dean to complete the Institution Evaluation Form, and should it be required, a separate hard copy Dean’s Letter of Recommendation. If you also require a Dean’s Letter of Recommendation, please indicate that on the form, and provide the addresses of the institutions.

Before you start:

  • Download, complete, and sign the Information Release Form – the signed form must be submitted before we can begin processing your request.
  • Print a PDF version of your NBDE results by clicking here, logging in to your account, and selecting the “display” option under the “dates tested” header.

** Your Dean’s Letter will be submitted by the official program deadline. The Dean’s Office will not begin to process your request without the completed form below and all required supplemental documentation. **
