Finding Faith Today Survey

The Finding Faith Today Survey

Informed Consent Form

“Finding Faith Today” Research Project
Conducted by Dr. Bryan Stone,
Boston University School of Theology

Thank you for considering participation in this research study as an individual who has recently (in the last ten years or so) made a religious/spiritual commitment, profession of faith, or conversion. We recognize that for some persons this new commitment will be more like a “return” or activation of the faith/religion into which they were born and that how one describes this new commitment, membership, or conversion will vary from tradition to tradition and person to person. Sometimes the language of conversion is inadequate to describe the way some people take up religious practice, for example.

This survey forms part of a national project called “Finding Faith Today” being carried out to look at people’s behavior and attitudes in relationship to their new profession of faith, commitment, or conversion. All the information you give will remain completely confidential. The purpose of this study is to gain a better understanding of people’s journey toward faith. The results of this research will be the eventual publication of a report (likely in book form) that summarizes and explores what is learned from the surveys.

There are minimal or no risks and benefits involved in your participation in this research. You have the right to withdraw from the study at any time by choosing not to complete the questionnaire.
• You may stop at any time or skip over a question if you find it upsetting.
• There are no costs to you for participating in this research study except for your time. I estimate that it will take you approximately 10-15 minutes to complete this questionnaire.
• You will not be paid to participate in this research study.

Confidentiality
All of your responses will be kept anonymous and confidential. Information from this study and study records may be reviewed and photocopied by the institution and by regulators responsible for research oversight such as the Office of Human Research Protections and the Boston University Institutional Review Board.
