Relevant Paperwork and Forms
- Continuing study form: Students should register for continuing study in fall or spring if they are on an internship or no longer taking courses for credit. Students are required to be registered every semester (fall, spring, and summer).
- Summer registration form: Students should register for the summer, using this form. Students are required to be registered every semester (fall, spring, and summer).
- Report of examination form: Students must complete this form for their qualifying exams.
- Dissertation prospectus approval form: Students must complete this form for their dissertation prospectus.
- Dissertation defense report of examination form: Students must complete this form for their dissertation defense.
