Submit Your Forms

Student forms are available for download below. Some forms are required, and some are intended only for applicable students. The completion and postmark deadline for Required Forms is May 20, 2025. If you have any questions about submitting forms, please contact us, and we’ll be happy to assist.

Required Forms (All Students)

Part I: Electronic Forms (three sections)

Each of the three electronic forms below must be filled out and submitted individually, and may only be submitted once. You will need your Identification Code (located in your admissions offer email) to complete these forms.

Part II: Student Life Packet (to print and mail)

The Student Life Packet must be printed, completed, and mailed to BUTI.

Chamber Music Form

  • Chamber Music Form | Please fill out the form below to record your chamber music experience and preferences. This form should only be filled out if you are attending the Young Artists Orchestra, Young Artists Wind Ensemble, or Young Artists Piano Program.

Student Absence Request

  • Student Absence Request In cases where a student will be absent from academic obligations and/or absent overnight, please complete this Absence Request Form in advance of the requested absence.

Lessons Request

  • Lessons Request | Use this form to request additional lessons not provided in your program(s) of study. Although this form is usually used by instrumentalists in our ensemble programs, any student can fill out this form to request additional lessons.
