Preparation for your First Visit

Intake Paperwork

To save time, please fill out the Intake Forms prior to your first appointment.

What to Bring

  • Completed paperwork
  • Prescription for physical therapy
  • Photo ID
  • Insurance card
  • HSA/HRA Information
  • Change of clothes if needed
  • Pertinent diagnostic images (X-ray, MRI, etc.)
  • List of current medications (patients with Medicare)

What to Wear

It is best to wear comfortable clothing for your visit that allows the therapist to assess your injury. Shorts are best for lower body concerns, and for upper body/neck problems, a tank-top is best. If your problem is related to running, bring your running shoes so the therapist can assess you in motion.

Directions and Parking

We are located at 915 Commonwealth Avenue. Our entrance is in the rear of the building. If you are using a GPS for navigation, some find it helpful to enter 10 Buick Street. Please also see our directions page or call the office at 617-358-3700.

Located behind our building, there is reserved parking in Lot A. Please tell the attendant you are here for an appointment at the Ryan Center. Parking is $2/hr. We do not validate.

What to Expect During your Evaluation

Once you have arrived at the BU Physical Therapy Center, check in with the front desk. Plan to have your IDs and prescription with you. We will ask you to complete paperwork if you have not done so already.

Your first visit will be an evaluation where your therapist will begin the assessment of your current problem. Your therapist will ask about your history, injury, pain, and functional goals for treatment. Together you will set goals, agree on a plan of care, and potentially start treatment.

Your treatment plan will be determined based on several factors including your examination findings, current evidence, past experience with treatment, and your doctor’s prescription.

Duration of Visit

Your evaluation will take 30-60 minutes. If you do not have the chance to complete intake paperwork prior to your visit, please arrive 15 minutes early to do so.

Student Involvement in Your Evaluation

You will be evaluated and treated by a licensed physical therapist 100% of the time. As a part of Boston University Sargent College, our mission includes providing clinical experiences for students in BU’s Doctor of Physical Therapy program. Your care is always directly supervised or performed by a licensed physical therapist. Every patient has the right to deny student involvement, and our therapists will seek that permission at your first visit.
