Student-Athlete Requirements
All students participating in Varsity (NCAA) athletics and select Club Sports must complete certain health requirements to be eligible for participation. These are in addition to the requirements for all BU students and must be completed each year they are a student athlete. Please complete your requirements as soon as possible to avoid delays in your eligibility to participate.
You are required to complete Student Athlete Health Requirements if you are:
- Participating in any Varsity (NCAA) athletics
- Participating in the following Club Sports: Ice Hockey, Rugby, or Soccer
Requirements
Athletic Screening Questionnaire
This provides information about your medical background as it relates to your sport. You don’t have to complete it all at once. You can select “Save Partial” to save your progress and come back later.
Complete the Questionnaire:
- Log in to Patient Connect
- On the left-hand side, select Medical Clearances
- Select Athletic Screening Questionnaire – Incoming or Athletic Screening Questionnaire – Returning*
- Complete all of the fields
- Select Submit
- Wait for the confirmation message that your questionnaire has been submitted
*The returning questionnaire is specific to student-athletes that have already participated at BU for a year and not for transfers that have participated at a previous institution.
Health Insurance Card
Please provide a copy of your health insurance card.
Upload Your Card:
- Take photos of the front & back of your insurance card
- Log in to Patient Connect
- On the left-hand side, select Insurance Card
- Select Add New Card
- Input your insurance information into the text boxes
- Select Upload Front Image to upload the photo of the front of your insurance card
- Select Upload Back Image to upload the photo of the back of your insurance card
- Select Save
SWAY Baseline Concussion Test
The SWAY Baseline test only needs to be completed once as an incoming student athlete. You will receive an email regarding completing the SWAY baseline test prior to the start of your season.
Pre-Participation Examination
These examinations ensure that you’re medically cleared to participate in your sport. You will receive an email when Pre-Participation Examination appointments are available to schedule prior to the start of your season.
Additional Requirements for Varsity Athletes, ONLY:
ADHD Documentation
We require documentation of Attention Deficit Hyperactivity Disorder (ADHD) diagnosis and prescriptions for stimulant medication if a Varsity student-athlete is selected for drug testing by the NCAA.
If you are prescribed a stimulant medication for ADHD:
- Complete the ADHD Documentation Form with the help of your prescribing physician and a parent/guardian
- The following supporting documentation must be provided along with the Form:
- Clinical evaluation notes referencing the use of DSM-IV guidelines
- Results from any ADHD Rating Scales [i.e., Conners, ASRS, CAARS, etc.]
- Consideration of non-stimulant medications and reasons they were not utilized
- Blood pressure and heart rate readings and analysis
- Follow-up orders for the patient
- Any other pertinent information regarding diagnosis, if available

- Select New Message
- Select Athletic Training Department (Varsity/Club Athletes)
- Select Submit ADHD form and documentation
- Select Add attachment and follow your device’s instructions to upload your documentation
- Compose your message in the text box
- Select Send
Sickle Cell Testing Results
Varsity student-athletes must submit documentation of a hemoglobin solubility test before competing at any NCAA institution.
Newborn sickle cell screening is not an acceptable form of documentation of sickle cell status. There is a fee associated with obtaining a hemoglobin solubility test and this cost is the responsibility of the student-athlete. Find options for obtaining a Sickle Cell Test in our FAQs below.
Upload Testing Results:
- Log in to Patient Connect
- On the left-hand side, select Messages
- Select New Message
- Select Athletic Training Department (Varsity/Club Athletes)
- Select Submit Sickle Cell Test Results
- Select Add attachment and follow your device’s instructions to upload your results
- Compose your message in the text box
- Select Send
