Health History

As part of your health requirements, you must complete a Health History with SHS. Sharing your health history lets us know about your medical background and helps us provide you with better care.

How to Complete your Health History

The Health History section is completed within our online portal, Patient Connect. Please complete the Health History section one month before the start of your first semester.

We recommend that you gather your information before you log in to more easily complete the Health History section. You can save the form at any time and come back to it. You will be asked to provide information on:

• Medical and mental health issues you’ve experienced
• Surgeries
• Allergy information
• Medications you’re currently taking
• Relevant family health information
• Relevant personal information
• Emergency contact information

When you’re ready to complete your Health History, follow these steps:

1. Log in to Patient Connect
2. Select Health History tab
3. Update the sections to the best of your ability
