Flu Clinics Begin for Students, Residents, and Fellows

A flu vaccination clinic will be held for students, residents, and fellows on Friday, November 9, 2012, from 11 a.m. to 4 p.m. in room G-309. Boston University Student Health Services will be on site to administer vaccines. Bring your BU ID to receive the vaccine.

Please be advised that if you receive your flu vaccine during this on-site clinic, the cost of the immunization will be covered by GSDM. If you choose to receive the flu vaccine from an outside provider, you will be responsible for the cost of the vaccine.

If you have already received the vaccine, or wish to decline the vaccine, you must complete a flu reporting form, as per the Massachusetts Department of Public Health (MDPH). MDPH regulation requires all licensed health care facilities to report information to MDPH documenting compliance with the vaccination requirement, in accordance with reporting and data collection guidelines of the Commissioner [105 CMR 130.325(I), 140.150 (C), 150.002(D)(8)].

Flu reporting forms can be obtained in Student Affairs in G-305 or at the on-site clinic.
