Process Overview
The Equal Opportunity Office (EOO) is here to facilitate the interactive dialogue between employees and their supervisors and explore reasonable accommodations in the workplace. If you are a student, Disability and Access Services will be able to assist you should you require accommodations.
Request
When we receive an accommodation request or referral, we email the employee or applicant to gather supporting medical documentation, depending on the nature of the request. You can find the medical provider form here: Request for Medical Information for ADA Accommodations. We recommend having the treating provider most familiar with your needs complete this form.
Engage
Once a request and supporting documentation are received, the EOO engages promptly in an interactive dialogue with the requesting individual and their direct supervisor to discuss the reasonableness of the request. All parties are expected to participate in this process in good faith. Please note: While the University considers an employee’s preference when assessing reasonable accommodation requests, we may offer an alternative accommodation if it is effective.
Implement
If an accommodation is approved, both the employee and their supervisor will receive a copy of the formal agreement. This document outlines the details of the accommodation, including how long it will be in place and whether it will be reassessed in the future.
For accommodations that include a reassessment, the EOO will check in at regular intervals to make sure the arrangement continues to meet the needs of both the employee and the department.
