Pre-departure forms

Below are the pre-departure forms that each student is required to complete.

1. Housing Questionnaire (All programs except London/Paris Architecture)

2. Course Selection Form (Fall & Spring internship program)  Course Selection Form (Summer internship)

3. Your travel arrangements (All Programs)

4.  Passport-size photo of yourself (All Programs)
Email a passport-size photo of yourself as a JPG attachment to buparis@bu.edu.  The name of the file should be your name (last, first) and semester of study in the format “Duck, Donald, S12.” Boston University students may scan and email their photos free of charge at BU Common @ Mugar. Non-Boston University students should consult the computer and technology support office at their home schools.
