Welcome. In the following grouped categories, you will find the forms you need to proceed through the Graduate Program in Bioinformatics’ M.S. and P.h.D programs. If you are unclear about which form(s) you need to complete a process, or if you cannot seem to locate a certain document on this website, please contact David King.
General
Graduate School of Arts & Sciences Procedures and Forms
Includes forms and instructions related to: MS and Ph.D. Diploma Applications, Dissertation Prospectus Approval and Submission, Submission of Dissertation Defense Abstract and Scheduling the Final Oral Examination (Dissertation Defense)
Required Forms: Lab Rotations and Advisors
Note: Some of these documents are PDFs and require a reader like Adobe’s Acrobat Reader to view them.
Students must fill out these forms and submit them to Dave King.
- Committee Approval Form
Students must have their examination committee as well as the date/time of their oral examination approved via a Committee Approval Form. This form must be submitted to Dave King at least one month in advance of the oral examination. The oral examination cannot be scheduled any less than a month from when the Committee Approval Form is submitted. Specific guidelines for choosing your committee are on pages 8 and 9 of the Student Handbook. Please not that your Research Advisor(s) cannot chair the committee and also that any committee member that is from another institution or an outside agency requires a Special Service Appointment Form. This form is available at GRS or in the Bioinformatics Department. This form only has to be used for a dissertation committee when there is a change from the original committee. - Research Advisor Election/Change Form
You must complete this form in the event that you want to change your advisor. Changes made without the use of this form will not be recognized. - Lab Rotation Approval Form

Completed forms must be turned in by October 1st of every year in order to gain permission to register for the following semester. ‘Complete’ means that a copy of your transcript must be attached and it must show all required signatures.
Classes, Student Status and Credit Transfer
- Continuing Student Status Registration Form (For Full-Time Students) Use this form if you’re not registering for any credit and need to have continuing student status.
- Petition for Approval of Credits for Transfer
- Certificate of Full-Time Participation in Graduate Program Use this form only when you’re registering for credits and need to be certified full-time. This is not the form to use if you’re not registering for any credits and need to be certified full-time; that is the form below.
- Petition Form Use this form to use one course in lieu of another or to request a waiver for requirement. Please be sure to attach any relevant course decriptions.
