Spring 2026 leave of absence/withdrawal official request form

The leave of absence / withdrawal official request form below is for use only by continuing undergraduate degree candidates who have thoroughly reviewed the linked information at www.bu.edu/usc (including the FAQs) and are sure they wish to arrange a Spring 2026 leave or withdrawal.

  • Metropolitan College students, graduate students, and non-degree, visiting, non-matriculated, and summer guest students should not use this form.  Instead, follow the leave of absence / withdrawal / course drop processes as outlined by these respective academic areas.
  • Incoming first-year and transfer undergraduate students for Spring 2026: if you cannot attend this semester as planned, email admissions@bu.edu.

Return from leave semester: there are three different options – Summer (May 2026), Fall (September 2026), and Spring (January 2027)    

Leave of absence/withdrawal reasons:  When filling out the leave/withdrawal reasons text box, please indicate the primary reason for your leave/withdrawal request (select from the choices listed below).  You can then elaborate on your primary reason, and/or note any secondary or additional reasons, as well.

Tuition and fees are canceled in accordance with the University’s withdrawal and tuition refund / cancellation schedule.
