First Visit
When you come to see us for the first time, please be sure to bring with you:
- a CD with copies of your chest x-rays or CAT scans,
- copies of previous pulmonary function tests (PFTs), and
- copies of notes written by your primary care doctor or pulmonologist.
To help us provide the best possible care, we request that you also complete the form below and bring it to your first visit to the Alpha-1 Center:
