Online registration will be conducted to reduce the duration of the registration process. All online team and coach registration must be completed by Friday, January 23 in order to compete. The forms listed below must subsequently be completed and mailed in hard copy format no later than Friday, February 6, to the following address:

Coaches must turn in an Adult Privacy Release form and a Medical Information Form.

All attending (including those not participating) students must turn in Parental Consent/Privacy Release Form and a Student Medical Form. If the student is 18 years of age or above, he/she need not require a parental signature.
