The Commonwealth of Massachusetts currently plans to start Phase 2 of its vaccine distribution timeline in February 2021. The initial priority groups in Phase 2 include:
- individuals age 75 and over;
- individuals age 65 and over;
- individuals with two or more high-risk (comorbid) health conditions that increase the risk of severe illness from COVID as identified by the CDC (please read the state’s list of high-risk conditions below); and
- residents of low-income senior housing and affordable senior housing.
Later in Phase 2, individuals with one or more high-risk health conditions will also be eligible for vaccination, among many others. See the Commonwealth’s priority list and BU’s chart of Phase 2 priority groups.
As BU prepares for Phase 2, we want to request the appropriate number of vaccine doses from the Commonwealth for members of our community who are eligible for vaccination in Phase 2. While we will identify students who are eligible due to age, we need your help to identify individuals with high-risk health conditions or who live in low-income and affordable senior housing.
If you have one (or more) of the high-risk conditions below, we invite you to visit Patient Connect to complete a brief survey and the appropriate attestation. You do not need to complete this survey if you do not have any of these conditions.
This survey must be completed by Friday, February 5, to be included in BU’s request for Phase 2 vaccine. Failure to respond by this date may delay receiving your vaccination through BU.
It is voluntary for you to provide this information. The information will only be used to request the appropriate number of vaccine doses for Phase 2 and to form the Phase 2 vaccination list. You will not be asked to specify your specific health condition(s). Your response will be stored in your confidential Student Health medical record.
NOTE: The Commonwealth’s vaccine distribution timeline for Phase 2 and allotment of vaccines to BU could be delayed. Furthermore, completion of this survey does not guarantee an immediate vaccination appointment at BU. BU will send you an email inviting you to schedule your vaccination appointment through Patient Connect when we are able to offer the vaccine to you. If you receive your COVID vaccination(s) elsewhere, please upload your vaccination documentation to Patient Connect.
