How do you write orders so that Bill can have the melatonin?

After discussion the risks and benefits of melatonin with Bill and his mom, you agree to write a medical order so his nurse can dispense it to Bill at the bedside. You go to the order set. Here are your choices.

Answer

Choice 1

Melatonin - Follow the instructions on the bottle.

INCORRECT. Just like writing medication orders you have to specify the exact dose, route, and timing of the dietary supplement

Choice 2

Patient may take home supply of Melatonin after it has been reviewed by the pharmacy. Melatonin will be stored in the locked medication pixis and dispensed by a nurse.

Melatonin (5mg) Administer 5mg po qhs PRN reason for sleep

CORRECT!

To write a dietary supplement order, follow the same technique you use for prescription medications. Remember you have to specify in the order that the patient has permission to use their home supply. Every hospital has its own unique protocol. Find out what is done in your hospital.

Choice 3

Patient may store Melatonin at the bedside

INCORRECT. Under the Joint Commission guidelines, all dietary supplements should be stored in the locked storage or pixis - not at the bedside.
