Appointment Request Form: Student Appointments

Please use the below form to submit a request for the appointment of a student to be an instructor of record, teaching assistant, field supervisor, or graduate assistant.

If you would like to request a part-time appointment for a non-student, please follow the instructions on this page.

Note: Please be aware that students may not work more than 20 hours per week for the university when classes are in session.

All requests will be reviewed by associate deans as appropriate, and requestors will be notified of approvals as soon as possible.
