Again these are created                   with the <code>&lt;input&gt;<\/code> tag, this time with TYPE=&#8221;radio.&#8221;                   Giving multiple buttons the same NAME will again define a single                   form field, but selecting different buttons will deselect previously                   selected buttons. Again be sure to include VALUE attributes.<\/p>\n<p>For example:<\/p>\n<blockquote><p><code>My affiliation is: &lt;br&gt;<br \/>\n&lt;input type=\"radio\" name=\"affiliation\" value=\"faculty\"&gt;                   Faculty &lt;br&gt;<br \/>\n&lt;input type=\"radio\" name=\"affiliation\" value=\"staff\"                   checked&gt; Staff &lt;br&gt;<br \/>\n&lt;input type=\"radio\" name=\"affiliation\" value=\"student\"&gt;                   Student &lt;br&gt;<br \/>\n&lt;input type=\"radio\" name=\"affiliation\" value=\"other\"&gt;                   Other &lt;br&gt; <\/code><\/p><\/blockquote>\n<p>Produces the following form.
