A Multiple Choice field will be added to your form.

Hover over the form and click the Edit button.

Under the Properties tab, enter the Field Label. This is where you will put the question you want answered by the choices you will add in the next step.

Gravity Forms will automatically add three choices. Edit these fields by clicking in the box that says First Choice and type in your choice. Repeat this process for each choice you want to add. If you need more than the three available fields, click the plus (+) button to add an additional choice. A required field will prevent the form from being submitted if it is not filled out or selected.

Check the No Duplicates box to limit user input to unique values only. This will require that a value entered in a field does not currently exist in the entry database for that field.

Click the Close button at the top of the Field Editor to close the field edit window and continue to add or edit other fields.

When you have finished adding all fields, click the Save Form Button at the bottom of the page.

Setting up Email Notification >>

The Add Fields tool allows you to browse the types of fields you may want to add to your form.
