Your certificate choice triggers advisor assignment. tab.<\/p>\n<p><\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h6 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">How do I obtain proof of enrollment?<\/h6><div class=\"bu_collapsible_section\" style=\"display: none;\"><\/p>\n<p>You may request an enrollment verification from our office\u2014the Verification Request Form is available on our <span><a href=\"http:\/\/www.bu.edu\/sph\/forms\">forms<\/a><\/span> page. A Verification Request Form is required for all requests, you may additionally submit any form you need completed for a third-party.<\/p>\n<p><\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h6 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">How do I change my legal name?<\/h6><div class=\"bu_collapsible_section\" style=\"display: none;\"><\/p>\n<p><span>At this time, Boston University\u2019s student information system, transcript, diploma, and degree verifications reflect an individual\u2019s legal name. This means that all official documents related to student records at Boston University reflect the legal name of an individual. An individual\u2019s legal name is defined as the name that identifies an individual for legal and official purposes. Legal name identifies an individual on official government records such as U.S. social security cards, state licenses, passports, and marriage or divorce decrees.
