Massachusetts<\/em><em> <\/em>affirmed the authority of states to steward public health. Since then, state legislatures and public health agencies have held primary responsibility for enacting and enforcing laws and policies to protect the health of their citizens, while the federal government has largely played a supportive role with agencies like the Centers for Disease Control and Prevention lending expertise and financial assistance.<\/p>\n\n\n\n<p>On April 16, nine School of Public Health faculty shared their expertise in a briefing to Massachusetts state senators and representatives from the&nbsp;<a href=\"https:\/\/malegislature.gov\/Committees\/Detail\/J16\" target=\"_blank\" rel=\"noreferrer noopener\">Joint Committee on Public Health<\/a>. Over the course of the two-hour briefing, the faculty from various departments across SPH delivered 15-minute presentations on a range of topics pertinent to the health of Massachusetts residents.<\/p>\n\n\n\n<p>\u201cThroughout history, the federal government has been a reliable partner to states in increasing resources for healthcare and public health. To have that spigot turned off without warning, I think is unique in our history,\u201d said <a href=\"https:\/\/www.bu.edu\/sph\/profile\/nicole-huberfeld\/\">Nicole Huberfeld<\/a>, Edward R. Utley Professor of Health Law, Policy &amp; Management. The briefing was the second such session to be held on the BU Medical Campus since 2023.<\/p>\n\n\n\n<p>Rep.&nbsp;<a href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/MCD1\" target=\"_blank\" rel=\"noreferrer noopener\">Marjorie Decker,<\/a>&nbsp;committee chair of house members and an instrumental force in organizing both briefings, noted that approximately 90 percent of her colleagues are new to the committee and expressed gratitude for SPH\u2019s continued partnership.<\/p>\n\n\n\n<p>\u201cWe&#8217;re certainly in state legislature at a time in which we&#8217;re going to struggle together\u2026. You are probably very familiar with a lot of the cuts that have come down that will impact public health in Massachusetts but specifically also DPH,\u201d said Decker, referring to the Trump administration\u2019s <a href=\"https:\/\/www.mass.gov\/news\/healey-driscoll-administration-condemns-president-trumps-termination-of-11-billion-in-public-health-grants\" target=\"_blank\" rel=\"noreferrer noopener\">recent termination of $11 billion<\/a> in federal grants to state public health agencies, including millions for behavioral health care, disease prevention and treatment, public health workers, community health centers, and laboratory operations in Massachusetts. \u201cWe will have to use this lens in trying to figure out what we can advance, what\u2019s feasible and what&#8217;s not, and what the consequences are and what the workarounds might be.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-embed-youtube alignwide wp-block-embed is-type-video is-provider-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<div class=\"responsive-video responsive-youtube\"><iframe loading=\"lazy\" title=\"SPH Faculty Brief Massachusetts Legislators on State&#039;s Public Health Priorities\" width=\"500\" height=\"281\" src=\"https:\/\/www.youtube.com\/embed\/Do1vvWQLxUg?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe><\/div>\n<\/div><\/figure>\n\n\n\n<p>While <a href=\"https:\/\/wallethub.com\/edu\/states-most-least-dependent-on-the-federal-government\/2700\" target=\"_blank\" rel=\"noreferrer noopener\">Massachusetts relies less<\/a> on federal funding than many other states, the Commonwealth has frequently leveraged federal dollars for its public health agenda, including to gain its first-in-the-nation status for <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2888014\/\" target=\"_blank\" rel=\"noreferrer noopener\">expanding health care coverage<\/a> to nearly all citizens in 2006.<\/p>\n\n\n\n<p>\u201cA lot of the questions that you are trying to answer in your political lives, I imagine, are [around] what do we do without that [money]? How do we move forward if we have to fund ourselves?\u201d said Huberfeld. She advised Decker and her colleagues aim to act in ways that will \u201creduce the risk of illness, injury, and death based on available evidence,\u201d and suggested that the committee focus their energy and resources on the social determinants of health or, in other words, the upstream factors that affect a person\u2019s health, such as their living and working conditions. Her recommendation would be echoed by several other SPH faculty presenters.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.bu.edu\/sph\/profile\/madeleine-scammell\/\">Madeleine Scammell<\/a>, associate professor of environmental health, commenced the event with her presentation on the role of local public health departments; <a href=\"https:\/\/www.bu.edu\/sph\/profile\/andrew-stokes\/\">Andrew Stokes<\/a>, associate professor of global health, talked about cardiovascular disease and GLP-1 medications; and <a href=\"https:\/\/www.bu.edu\/sph\/profile\/lois-mccloskey\/\">Lois McCloskey<\/a>, professor of community health sciences and director of the <a href=\"https:\/\/www.bu.edu\/mchcenterofexcellence\/\" target=\"_blank\" rel=\"noreferrer noopener\">BUSPH Center of Excellence in Maternal &amp; Child Health<\/a>, and <a href=\"https:\/\/www.bu.edu\/sph\/profile\/kathryn-thompson\/\">Kathryn Thompson<\/a>, assistant professor of community health sciences, discussed maternal health.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.bu.edu\/sph\/profile\/carlos-rodriguez-diaz\/\">Carlos Rodriguez-Diaz<\/a>, chair and professor of community health sciences, warned the legislators of the potential for worsening health outcomes among the LGBTQ+ community in Massachusetts. He predicts that given the changes in policies affecting the LGBTQ+ population nationally, Massachusetts\u2019 more progressive laws will\u2014and likely already have\u2014spur migration to the state. He urged incorporation of \u201chealth in all policies,\u201d highlighting the ongoing health disparities faced by the LGBTQ+ community including higher rates of mental health issues, substance use disorder, and certain physical health problems that often stem from stigma, discrimination, and a lack of access to culturally competent healthcare. Rodriguez-Diaz highlighted two studies undertaken by his SPH colleagues to better understand and address the drivers of these inequities: <a rel=\"noreferrer noopener\" href=\"https:\/\/www.wholeselv.es\/\" target=\"_blank\">The Whole Selves Project<\/a> on healthy relationship building for trans and nonbinary youth and the <a rel=\"noreferrer noopener\" href=\"https:\/\/sites.bu.edu\/heylab\/what-we-do\/spectrum\/\" target=\"_blank\">Studying Policies\u2019 Effects on Communities: Trends in Resilience and Undergraduate Mental Health (SPECTRUM) Study<\/a>. Unfortunately, funding for both was recently terminated, he informed the legislators.<\/p>\n\n\n\t<aside class=\"wp-block-editorial-relatedstories is-style-list alignright sphnews-block-editorial-relatedstories\">\n\t\t<h3 class=\"wp-block-editorial-relatedstories-title\">Related<\/h3>\n\t\t<ul class=\"wp-block-editorial-relatedstories-list\">\n\t\t\t\t\t\t\t<li class=\"wp-block-editorial-relatedstories-list-item\">\n\t\t\t\t\t<article class=\"wp-block-editorial-relatedstories-article\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<p class=\"wp-block-editorial-relatedstories-article-category\"><span>School News<\/span><\/p>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<h4 class=\"wp-block-editorial-relatedstories-article-title\"><a href=\"https:\/\/www.bu.edu\/sph\/news\/articles\/2023\/we-are-all-lifelong-learners-in-this-space\/\" class=\"wp-block-editorial-relatedstories-article-title-link\">SPH Hosts Briefing for Massachusetts Joint Committee on Public Health<\/a><\/h4>\n\t\t\t\t\t\t\t<p class=\"wp-block-editorial-relatedstories-article-date\">May 5, 2023<\/p>\n\t\t\t\t\t\t\t\t\t\t\t<\/article>\n\t\t\t\t<\/li>\n\t\t\t\t\t\t<\/ul>\n\t<\/aside>\n\t\n\n\n<p>In interim Dean <a href=\"https:\/\/www.bu.edu\/sph\/profile\/michael-stein\/\">Michael Stein<\/a>\u2019s opening remarks, he also drew the legislators\u2019 attention to disparities in U.S. spending priorities. \u201cAmerica spends $11,000 per person on medical care and $200 per person on public health,\u201d he said, citing 2019 <a href=\"https:\/\/www.cdc.gov\/nchs\/fastats\/health-expenditures.htm\" target=\"_blank\" rel=\"noreferrer noopener\">CDC<\/a> and <a href=\"https:\/\/healthcostinstitute.org\/hcci-originals-dropdown\/all-hcci-reports\/spending-on-preventive-services-represents-a-small-fraction-of-total-health-care-spending-but-costs-to-individuals-could-be-high-without-aca-protection#:~:text=In%202019%2C%20total%20spending%20on%20preventive%20services,on%20health%20care%20services%20over%20the%20year.\" target=\"_blank\" rel=\"noreferrer noopener\">Health Care Cost Institute<\/a> data. \u201cThat suggests that public health is [less than] one fortieth as important as medicine. We here don&#8217;t think that.\u201d<\/p>\n\n\n\n<p>In total, nine legislators participated in the briefing, including Sen. <a rel=\"noreferrer noopener\" href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/WJD0\" target=\"_blank\">William J. Driscoll, Jr.<\/a>, chair of senate members; Rep. <a rel=\"noreferrer noopener\" href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/SPK1\" target=\"_blank\">Sally P. Kerans<\/a>, vice chair of house members; Rep. <a rel=\"noreferrer noopener\" href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/BJA1\" target=\"_blank\">Bruce J. Ayers<\/a>, Rep. <a rel=\"noreferrer noopener\" href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/M_C2\" target=\"_blank\">Michelle L. Ciccolo<\/a>; Rep. <a rel=\"noreferrer noopener\" href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/HEK1\" target=\"_blank\">Hannah Kane<\/a> (Questrom \u201893); Rep. <a href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/S_R1\">Sean Reid<\/a>; Rep. <a href=\"https:\/\/malegislature.gov\/Legislators\/Profile\/AMS3\">Amy Mah Sangiolo<\/a>; Rep. Decker; and Rep. Monta\u00f1o. <a rel=\"noreferrer noopener\" href=\"https:\/\/www.linkedin.com\/in\/ramla-hagi-9614b4153\/\" target=\"_blank\">Ramla Hagi<\/a>, the committee research director and an incoming SPH student; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.linkedin.com\/in\/daniella-montero\/\" target=\"_blank\">Daniella Montero<\/a> (SPH \u201823), a research analyst with the committee and and an SPH alum; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.linkedin.com\/in\/sarahpierson16\/\" target=\"_blank\">Sarah Pierson<\/a>, the legal counsel to the committee; and <a rel=\"noreferrer noopener\" href=\"https:\/\/www.linkedin.com\/in\/ryantelingator\/\" target=\"_blank\">Ryan Telingator<\/a>, the chief of staff to Rep. Decker, also joined the event. <a href=\"https:\/\/www.bu.edu\/sph\/profile\/craig-andrade\/\">Craig Andrade<\/a>, associate dean for practice and director of SPH&#8217;s <a href=\"https:\/\/www.bu.edu\/sph\/practice\/activist-lab\/\">Activist Lab<\/a>, presided over the briefing schedule to ensure efficient use of the group&#8217;s time.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.bu.edu\/sph\/profile\/david-hamer\/\">David Hamer<\/a>, professor of global health and a member of the faculty of the <a href=\"https:\/\/www.bu.edu\/ceid\/\" target=\"_blank\" rel=\"noreferrer noopener\">BU Center on Emerging Infectious Diseases<\/a>, called the room\u2019s attention to the ongoing measles outbreak and other diseases of rising concern. He said that while Massachusetts is at low risk overall, given measles\u2019 very high reproductive rate, or the number of people who can become infected through contact with one infected person in the absence of immunity, the state must maintain its immunization rates.<\/p>\n\n\n\n<p>Public education and countering misinformation were recurring themes throughout the briefing, with many faculty emphasizing their importance.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.bu.edu\/sph\/profile\/monica-l-wang\/\">Monica Wang<\/a>, associate professor of community health sciences, closed the briefing with a presentation on communication strategies for public health. She noted that most people\u2014even many physicians\u2014do not know what the social determinants of health refer to and do not speak the language of public health terminology. The field should go beyond gathering facts and evidence to consider how to frame data in alignment with different audience\u2019s values and priorities, she said. She also raised the importance of trusted messengers, providing the example of <a href=\"https:\/\/www.usatoday.com\/story\/life\/humankind\/2021\/12\/09\/alabamas-dorothy-oliver-wins-best-humankind-award-vaccination-effort\/8889803002\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dorothy Oliver<\/a>, an Alabama retiree, whose door-to-door advocacy got her community over 95 percent vaccinated for COVID in 2021 when the overall state vaccination rate was less than 50 percent.<\/p>\n\n\n\n<p>The prevailing narrative is \u201cyou&#8217;re responsible for your own health,\u201d Wang said. \u201cThe work of public health and social determinants is the counter narrative, which is your environment matters, your social connections matter\u2014your income, your housing, your education, your race and ethnicity matter.
