I understand that I am borrowing Regalia belonging to the Graduate Student Life Office at Boston University School of Public Health. By signing this form, I agree to return ALL the regalia that is being so graciously loaned to me to the Graduate Student Life Office in Talbot 2 Center (Talbot Reception Desk) by Monday June 5, to allow future students the same opportunity that I have been given.

*Completing this form does not guarantee that you will obtain a set from the loaner regalia program. The only way to confirm your loaner regalia is when you arrive to pick up beginning Tuesday, March 14 starting at 1:00 pm from Talbot 211 East.
