Medical Records

A medical record is a history of your health and healthcare. Medical records can include information about your general health or identity, medical issues, test results, medications, surgeries, and more.

Students are entitled to copies of their medical records and can access them using a written authorization. Medical records are kept for 20 years after the student’s last visit or treatment at SHS.

How to Request Your Records

Current and former students can request their medical records online.

For medical or mental health records:

  • Complete the Authorization for Medical Release Form.
  • Within 3 weeks, you will receive copies of your records. You can choose to have them sent to you via secure email (DataMotion) or have them faxed to a specified location (such as a doctor’s office).

For immunization records:

  • Log onto Patient Connect
  • Under the “Immunizations” tab, your record is available to view and print

How to Amend Your Records

Students may request an amendment or clarification to their medical record if they believe information is inaccurate or incomplete. Requests can only be made up to two years after the documentation date.

Requests are reviewed thoughtfully and in accordance with applicable regulations. In some cases, if your record is determined to be accurate and complete, SHS may be unable to make the requested change. While SHS cannot remove or change factually accurate information, we recognize that language matters and are committed to using clinical documentation that is careful, respectful, and as clear and appropriate as possible.

To amend a medical record:

  • Complete the Request for Correction Form
  • In this form, you will be asked for the following information. To make the process easier, we recommend gathering this information in advance.
    • Medical Record Number
    • Date of service of the medical record
    • What information is inaccurate or incomplete
    • Why the information is inaccurate or incomplete
  • After completing the form, you will receive a copy. Your request will be processed within 30 days and you will receive a letter with the decision to either amend the record or a reason for denial will be provided.
