The following information may be useful to researchers in assessing individual risk and avoiding potential health problems.<\/p>\n<hr>\n<h2 id=\"evaluating-workplace-risks\">Evaluating Workplace Risks<\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>You can be at increased risk for infection when you are immunocompromised or pregnant.<\/b><span style=\"font-weight: 400;\"> Pregnancy is considered an immunocompromised condition. In addition, reproductive hazards are substances or agents that may affect the reproductive health of women or men or the ability of couples to have healthy children. Workers may be exposed to reproductive hazards by breathing them in (inhalation), by contact with skin (dermal), or by swallowing them (ingestion). In addition to identifying possible hazards in the workplace, EHS can help evaluate engineering controls, safety practices, and personal protective equipment (PPE) to minimize your risk of exposure. In general, safety practices in the research setting are designed to minimize all personnel exposure to hazards. Lab workers are urged to review the guidance provided at the link below as soon as they begin planning for pregnancy or believe they are pregnant.<\/span><\/p>\n<p><a href=\"https:\/\/www.bu.edu\/research\/ethics-compliance\/safety\/laboratory-safety\/laboratory-reproductive-health-hazards-guidance-document\/\" class=\"button button-base\">View Guidance<\/a><\/div>\n<h2 id=\"eliminating-reducing-workplace-risks\">Eliminating\/Reducing Workplace Risks<\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Make sure that you know the hazards associated with the material before handling them.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Complete your training and follow the Standard Operating Procedures (SOP) developed by your lab for working safely.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Always use the recommended engineering controls (such as Biological Safety Cabinets).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Always wear and use the required personal protective equipment.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Always wash your hands after contact with animals, potential hazards, and after taking off gloves.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ask for help in requesting accommodations in the workplace to avoid possible exposures.<\/span><\/li>\n<\/ul>\n<hr>\n<h2 id=\"infection-control\">Infection Control<\/h2>\n<p>Researchers should notify their health care provider of the type of work they are doing should they have any unexplained illness. In the case of allergies to laboratory animals, the proteins most frequently associated with the allergic reactions are found in the animal\u2019s urine, saliva, and dander.<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">What are the symptoms of allergic reactions to laboratory animals and when do they occur?<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">The earliest symptoms include nasal stuffiness, a \u201crunny\u201d nose, sneezing, red irritated eyes and hives. Symptoms that are particularly troubling are those that suggest the worker is developing asthma. These symptoms include coughing, wheezing and shortness of breath. Asthma resulting from allergic reactions to laboratory animals can result in severe and occasionally disabling breathing problems. Rarely, an employee with allergic symptoms will develop a potentially life\u2013threatening reaction following an animal bite.<\/p>\n<p>Most workers who develop allergic reactions to laboratory animals will do so within the first twelve months of working with them. Infrequently, reactions only occur after working with animals for several years. Initially, the symptoms are present within minutes of the worker\u2019s exposure to the animals. Approximately half of allergic workers will have their initial symptoms subside and then recur three or four hours following the exposure.<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">What laboratory animals are associated with allergic reactions?<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">Most animals used in research have been identified as the source of workers\u2019 allergy symptoms. Because mice and rats are the animals most frequently used in research studies, there are more reports of allergies to rodents than other laboratory animals.<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">What are the chances that a worker will develop an allergic reaction to laboratory animals?<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">It has been reported that one out of every three to five individuals who works with laboratory animals will develop allergic symptoms. Further, one in twenty workers with allergies to animal proteins will develop asthma as a result of their contact with laboratory animals.<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">Are there factors that are associated with an increased risk for developing an allergic reaction to laboratory animals?<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">Yes, a history of allergy to other animals (typically cats and dogs) is the best predictor for who will develop an allergy to animals found in research laboratories. Other factors associated with allergic reactions to laboratory animals include the individual\u2019s intensity, frequency and route of the exposure to the animals. Activities such as handling animals and cleaning their cages may be associated with an increased risk of exposure to the animal proteins and thereby place the worker at greater risk of developing an allergic reaction. Although workers who have a personal or family history for asthma, seasonal allergies and dermatitis are also at increased risk, individuals with no prior history of allergies and only brief work exposures can also develop allergic reactions to laboratory animals.<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">What can be done to reduce the chance that a worker will develop an allergic reaction to laboratory animals?<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">The best approach for reducing the likelihood that a worker will develop an allergic reaction is to eliminate or minimize their exposure to the proteins found in animal urine, saliva and dander. Ideally, this is accomplished by limiting the chances that workers will inhale or have skin contact with animal proteins. In addition to using well-designed air handling and waste management systems in research areas, workers can reduce their risk of exposure by routinely using dust\/mist masks, gloves and gowns.<\/p>\n<p>If additional respiratory protection is required, the worker should contact the Environmental Health and Safety Department, at (617) 638\u20138830 on Boston University Medical Campus (BUMC) or (617) 353\u20134094 on Charles River Campus (CRC). You must also complete an EHS <a href=\"https:\/\/www.bu.edu\/research\/forms-policies\/osha-respiratory-medical-evaluation-questionnaire\/\">ROHP OSHA Respiratory Medical Evaluation Questionnaire<\/a>. If you have any known or suspected animal allergies, please complete the <a href=\"https:\/\/www.bu.edu\/research\/forms-policies\/animal-allergy-screening-form-2\/\">Animal Allergy Screening form<\/a>. Completed forms should be submitted it to the Research Occupational Health Program (ROHP) at 72 E. Concord Street, Evans Building, 8th floor, Room 825.<\/p>\n<p><a href=\"http:\/\/ilarjournal.oxfordjournals.org\/content\/42\/1\/17.full?sid=980c9eed-44b9-4787-a7ea-87eb2a80cd42\">A 2001 article by D.J. Harrison explores ways to control exposure to laboratory animal allergens.<\/a><\/div>\n<\/div>\n\n<h3 id=\"cdc-summary-of-animal-biosafety-levels-absl\">CDC Summary of Animal Biosafety Levels (ABSL)<\/h3>\n<p>There are four increasing levels of containment that are called biosafety levels 1 through 4. Infectivity, severity of disease, transmissibility, and the nature of the work being conducted all play a role in how the CDC determines the level. <a href=\"https:\/\/www.cdc.gov\/labs\/bmbl\/\">An overview of each level is outlined on the CDC website.<\/a><\/p>\n<h3 id=\"working-with-nonhuman-primates\">Working with Nonhuman Primates<\/h3>\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">Working with Nonhuman Primates<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">It is important to recognize the risks that are present in such a work environment\u2026risks to the humans\u2026and risks to the nonhuman primates. For this reason, all protective safety\/health measures must be seriously adhered to.<\/p>\n<ul class=\"check-list\">\n<li>Employee protection in the nonhuman primate environment includes the use of protective, and in some cases, waterproof clothing.<\/li>\n<li>Shoe covers prevent transfer of pathologic organisms. Clothing and shoe covers are specific to the animal spaces but should be removed when outside of those areas.<\/li>\n<li>Combinations of goggles\/masks and\/or face shields protect the eyes, nose and mouth from splashes (see details below).<br \/>\nGloves are worn to protect contamination of the skin. In the circumstances when awake nonhuman primates will be handled, thick leather gloves specially designed for protecting against bites and scratches should be worn.<\/li>\n<li>Proper hand washing should be performed before and after wearing the protective clothes.<\/li>\n<\/ul>\n<p>Bites, scratches, and splashes of blood and body fluids from nonhuman primates pose numerous problems. Bacterial infections are the most frequent complication of a nonhuman primate bite. Cercopithecine Herpesvirus-1 or B Virus is a member of the herpes family occurring naturally in Macaque and possibly other old world monkeys. Most have no evidence of infection or others may have small blisters or ulcerations in the mouth, face, lips, genitalia, and\/or eye. Transmission to humans occurs by exposure to contaminated monkey saliva, secretions, or tissues. The most likely route is bites, scratches or splashes. The B Virus has never been cultured in blood. Injuries involving contact with nonhuman primate blood are potentially hazardous. Simian Immunodeficiency Virus (SIV) commonly persists without clinical manifestations in nonhuman primates. Type D Retroviruses (Simian retroviruses [SRV]) may infect some nonhuman primates. Alternatively, surgical assistants can wear safety glasses, face shield, and surgical mask.<\/p>\n<p><span class=\"label bg-darkgray\">Tracheal Tube Insertion and Removal during the beginning and end of NHP Surgery<\/span><\/p>\n<h6>Eye\/Face\/Mucous Membrane Protections:<\/h6>\n<p>Goggles and surgical mask.<\/p>\n<h6>Alternative, if available:<\/h6>\n<p>safety glasses (prescription if needed), face shield and surgical mask.<\/p>\n<p><span class=\"label bg-darkgray\">Perfusion of NHP at Termination Surgery<\/span><\/p>\n<h6>Eye\/Face\/Mucous Membrane Protections:<\/h6>\n<p>Completed in Fume Hood by Lead Surgeon: Involves slicing of heart ventricles, NHP anesthetized.<\/p>\n<h6>Alternative, if available:<\/h6>\n<p>Completed in Fume Hood by Lead Surgeon: Currently wears prescription eyeglasses and surgical loupes and surgical mask.<\/p>\n<p><span class=\"label bg-darkgray\">Magnetic Resonance Imaging of NHP Anesthetized Animal<\/span><\/p>\n<h6>Eye\/Face\/Mucous Membrane Protections:<\/h6>\n<p>Anesthetized animals in sealed microisolator cage, safety glasses and surgical mask should be worn.<\/p>\n<h6>Alternative, if available:<\/h6>\n<p>n\/a<\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">Working with Nonhuman Primate Tissue<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\">Employees working with nonhuman primate tissue have a more controlled environment, thus reduced risk for infection from nonhuman primate contact. Most contact is due to accidents in the laboratory. Therefore, it is recommended that universal precautions be practiced at all times. Thorough hand washing should be done before and after gloving. Nonhuman primate tissue should not be handled without gloves. Eye protection should be worn to prevent exposure from splashes. In some cases, clothing covers should also be utilized. All researchers working with nonhuman primate tissue must be evaluated and educated at an occupational health service.<br \/>\n&nbsp;<br \/>\nTissue, blood, and body fluids from nonhuman primates pose numerous problems. Cercopithecine Herpes virus-1 or B Virus is a member of the herpes family occurring naturally in macaque, rhesus, cynomolgus, and possibly other old world monkeys. It can be found in neurologic tissue, CSF, ocular, oral, or genital fluid secretions. B virus has never been cultured in blood. Due to the extreme morbidity and mortality of this infection in humans, special effort is taken to ensure prompt evaluation and first aid following potential exposure to B Virus.<br \/>\n&nbsp;<br \/>\nSimian Immunodeficiency Virus (SIV) commonly persists without clinical manifestations in nonhuman primates.<br \/>\n&nbsp;<br \/>\nType D Retroviruses (Simian retroviruses [SRV]) may infect some nonhuman primates. However, there is no current evidence on humans being infected with the type D retroviruses. Injuries involving contact with nonhuman primate blood are potentially hazardous. Each researcher working with NHP or their tissues is required to store serum for future reference.<\/p>\n<p><div class=\"important notice\"><div class=\"important-wrap\"><p> First aid for injuries related to exposure to nonhuman primates or their tissues is always handled as an emergency. <\/p><\/div><\/div><br \/>\nFirst aid should be performed before seeking medical care unless the injury is severe. See paragraphs below. Follow-up immediately with BU Research Occupational Health Program (ROHP) located at 72 E. Concord Street, Evans Building, 8th floor, room 825 and ROHP should be contacted immediately at (617) 414-7647. Hours are Monday through Friday from 8:00 am. \u2013 4:30 pm. On the Charles River Campus, contact and report to the BU Occupational Health Center (BUOHC) located at 930 Commonwealth Ave., West, Pleasant Street entrance, Tel: (617) 353-6630. On weekends, evenings, and holidays, report to the BMC Emergency Department, Harrison Avenue Campus, located at 840 Harrison Avenue, Menino Pavilion.<\/p>\n<p>For puncture and lacerations with a contaminated instrument: Cleanse the wound immediately with a Betadine sponge scrub for 15 minutes. Rinse thoroughly. Notify the employee\u2019s supervisor and the veterinarian responsible for the animal. Report to ROHP, BUOHC, or the BMC Emergency Department after initiating treatment for the full 15 minutes.<\/p>\n<p>For a splash of potentially contaminated fluid to the nose, mouth, or eyes: Flush the site for 15 minutes with water or saline solution. Notify the employee\u2019s supervisor and the veterinarian responsible for the animal. Purpose:<\/strong> To describe procedures for providing immediate care to personnel exposed to B-virus or Cercopithecine herpes virus 1 (formerly known as Herpes virus simiae or Herpes B.)<\/p>\n<p><strong>II. Introduction\/Background:<\/strong> A. Cercopithecine herpes virus 1 (CHV1), also known as B-virus, occurs naturally in macaque monkeys. The virus is enzootic among Old World macaques and usually causes minimal or undetectable morbidity in its natural host. Employees who have contact with these animals\u2019 tissues or secretions, may be exposed to B-virus through bites, scratches, cuts, contaminated needle-sticks and other contact of mucous membranes or broken skin with infected body fluids from macaques or with wet, unfixed tissues or primary cell culture material. In addition contaminated husbandry and research equipment can potentially spread B-virus.<\/p>\n<p>B. Persons exposed to contact with blood, tissue or secretions of nonhuman primates are at risk for B-virus infection through:<\/p>\n<ol>\n<li>Bites<\/li>\n<li>Eye splash<\/li>\n<li>Mucous membrane exposure<\/li>\n<li>Contaminated needle-sticks<\/li>\n<li>Exposure of cuts or broken skin to contaminated material.<\/li>\n<li>Scratch from an animal or cage\/equipment<\/li>\n<li>Exposure at necropsy (i.e., cut from a scalpel blade)<\/li>\n<li>Saliva contamination of an existing wound<\/li>\n<li>Exposure to infections tissue or fluids of NHP (i.e., , ocular, oral, genital secretions CNS tissue and CSF), that are potentially infectious.<\/li>\n<li>Exposure of broken skin to primary cell cultures de-rived from macaque kidneys are a potential source of virus.<\/li>\n<li>Human-to-human transmission (i.e., cleaning of a contaminated wound )<\/li>\n<\/ol>\n<p><strong>III. Pre-exposure Requirements for Employees<\/strong><\/p>\n<ol>\n<li>Contact Research Occupational Health for screening and approval for any work that may involve direct or indirect contact with non-human primates or non-human primate fluids, blood or tissue.<\/li>\n<li>Training is required for those working with NHP and NHP tissue: a.Classroom training: Herpes B virus and Working Safely with NHP training session b. If applicable, demo bite kit and eye wash stations tour c. Demonstration of PPE entry procedures d. Supplemental reading material may be provided; refer to attending veterinarian for questions e. Post- exposure instructions handouts provided during training sessions<\/li>\n<li>Personal Protective Equipment (PPE): a. Protective eyewear (e.g. goggles or glasses with solid side shields). b. Face Shield (must be able to prevent splashes to the head from running down into the eyes c. Face mask d. Disposable Lab coat +\/- Scrubs e. Disposable Jump Suits (if in street clothes or Quarantine room entry) f. Gloves ( 2 pairs ) g. Sleeve covers h. Shoe covers i. Hair bonnet<\/li>\n<\/ol>\n<p><strong>IV. Post-exposure Procedure Recommendations for <strong>Employees<\/strong><\/strong><\/p>\n<p><strong><em>Stop, Wash, Report<\/em><\/strong><\/p>\n<p><strong>A. Stop:<\/strong> If injured or exposed <strong>STOP<\/strong> what you are doing.<\/p>\n<p><strong>B. Wash:<\/strong><\/p>\n<ol>\n<li>IMMEDIATELY Scrub or irrigate wounds or scratches for 15 minutes contact time. a. Flush and scrub with chlorhexidine or Providine \u2013iodine surgical scrub brush and high volume of water for 15 minutes. Mechanical scrubbing of the wound margins is recommended in addition to the use of copious flowing water and chlorhexidine or Providine \u2013 iodine scrub to further cleanse the wound of contamination. (Deep wounds should be gently massaged to increase contact with cleaning solution). b. Mucous membrane exposures (eye, nose, or mouth): Irrigate or flush for 15 minutes with sterile 0.9 sodium chloride irrigation solution or sterile eye irrigating solution or water from eye wash stations (preferably at an eye wash station located in Labs or NHP animal facility). Never use any soap or disinfectants in eyes.<\/li>\n<\/ol>\n<p><strong>V. First Aid\/Post Exposure Prophylaxis<\/strong><\/p>\n<p>Perform one of the following actions:<\/p>\n<div class=\"responsive-table\"><table cellspacing=\"0\" cellpadding=\"0\">\n<tbody>\n<tr>\n<td valign=\"top\">Skin Exposure (Needlestick or scratch):<\/td>\n<td valign=\"top\">Immediately go to the sink and thoroughly wash the wound with soap and water for 15 minutes. Decontaminate any exposed skin surfaces with an antiseptic scrub solution.<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\">Mucous Membrane Splash to Eye(s), Nose or Mouth:<\/td>\n<td valign=\"top\">Exposure should be irrigated vigorously.<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\">Splash Affecting Garments:<\/td>\n<td valign=\"top\">Remove garments that may have become soiled or contaminated and place them in a double red plastic bag.<\/td>\n<\/tr>\n<\/tbody>\n<\/table><\/div>\n<p>For post exposure prophylaxis, individuals thought to be at high risk for infection following a bite, laceration, or puncture wound when working with infected neural tissue are administered antivirals. Prophylaxis is started as soon as possible after exposure (within hours), but only after wound cleaning has been completed. The time spent scrubbing or soaking the wound (15 to 20 minutes) is much more important for limiting\/preventing transmission, than the type of solution used. Three orally administered agents are currently available for post exposure prophylaxis of CHV-1 infection: acyclovir, valacyclovir, and famciclovir. The drug of choice is valacyclovir 1gram orally every 8 hours for 14 days.<\/p>\n<p><strong>VI. Treatment<\/strong><\/p>\n<p>If patient is symptomatic, intravenous antiviral therapy with acyclovir or ganciclovir is recommended until symptoms resolve and two cultures over 2 week period are negative. Treatment may be followed by long term suppressive therapy with oral antivirals.<\/p>\n<p><strong>VII. Baseline\/Day of Injury Samples<\/strong><\/p>\n<p><strong>A.<\/strong> Obtain 5-7ml red top tube for B virus serum testing. In the \u201c<strong>Miscellaneous Test Request<\/strong>\u201d section of the laboratory requisition; indicate the following tests to be completed: B \u2013Virus \u2013 coded as HBVSQ (blood test) and if open wound a swab culture for human virology coded as \u2013 HBVCQ.<\/p>\n<p><strong>B.<\/strong> A National B Virus Resource Center Viral Immunology submission form will need to be completed and included with the samples (addendum 3) or electronic copy can be obtained via <a href=\"http:\/\/biotech.gsu.edu\/virology\/PDFs\/2015%20Submission%20Form.pdf\"><span style=\"text-decoration: underline;\">http:\/\/biotech.gsu.edu\/virology\/PDFs\/2015%20Submission%20Form.pdf<\/span><\/a>). BMC Microbiology Supervisor will need to be contacted to inform them that a specimen is being sent over for the patient as well as the primate to be shipped via FedEx to the National B Virus state lab. The primates ID will need to be included in the National B Virus Resource Center Viral Immunology submission form. These specimens must be collected as close as possible to the time of injury, as specimens collected later may not accurately reflect the monkey\u2019s status at time of injury.<\/p>\n<p><strong>VIII.<\/strong> <strong>Follow-up\/14 &#8211; 21 Day Post Injury Samples<\/strong> Obtain 5-7ml red top tube for B virus serum testing.
