If you have additional questions, please contact us at 617-358-3700 or email <a href=\"mailto:buptc@bu.edu\">buptc@bu.edu<\/a>. <\/span><\/p>\n<h3><strong><span>Preparation for your First Visit<\/span><\/strong><\/h3>\n<h4><strong><span style=\"font-weight: normal !msorm;\"><span>Intake Paperwork<\/span><\/span><\/strong><\/h4>\n<p>To save time, please fill out the <a href=\"\/rehab\/files\/2026\/03\/2026-BU-Physical-Therapy-Intake-New-Patient-2.pdf\">Intake Forms<\/a>\u00a0<span>prior to your first appointment.<\/span><\/p>\n<h4><strong><span>What to Bring <\/span><\/strong><\/h4>\n<ul>\n<li><span>Completed paperwork- please complete digitally if possible<\/span><\/li>\n<li><span>Prescription for physical therapy<\/span><\/li>\n<li><span>Photo ID<\/span><\/li>\n<li><span>Insurance card<\/span><\/li>\n<li>HSA\/HRA Information<\/li>\n<li><span>Change of clothes if needed<\/span><\/li>\n<li><span>Pertinent diagnostic images (X-ray, MRI, etc.)<\/span><\/li>\n<li><span>List of current medications (patients with\u00a0Medicare)<\/span><\/li>\n<\/ul>\n<h4><strong><span>What to Wear<\/span><\/strong><\/h4>\n<p><span>It is best to wear comfortable clothing for your visit that allows the therapist to assess your injury. Shorts are best for lower\u00a0<\/span><span><\/span><span>body concerns, and for upper body\/neck problems, a tank-top <\/span><span>is <\/span><span>best. If your problem is related to running, bring your running shoes so the\u00a0therapist can assess you in motion.<\/span><\/p>\n<h4><strong>Directions and Parking<br \/>\n<\/strong><\/h4>\n<p>We are located at 915 Commonwealth Avenue. Our entrance is in the rear of the building. If you are using a GPS for navigation, some find it helpful to enter 10 Buick Street. Please also see our <a href=\"https:\/\/www.bu.edu\/rehab\/directions-to-buptc\/\">directions<\/a> page or call the office at 617-358-3700.<\/p>\n<p>Located behind our building, there is reserved parking in Lot A. Please tell the attendant you are here for an appointment at the Ryan Center. Parking is $2\/hr. We do not validate.<\/p>\n<h3><strong>What <\/strong><strong><span>to E<\/span><\/strong><strong><span>xpect <\/span><\/strong><strong><span>During your Evaluation<\/span><\/strong><\/h3>\n<p><span>Once you have arrived at the BU Physical Therapy Center, <\/span><span>check in with the front desk<\/span><span>. Plan to h<\/span><span>ave your IDs and prescription with you.<\/span> <span>We will ask you to complete paperwork if you have not done so already. <\/span><\/p>\n<p>Your first visit <span>will be an <\/span>evaluation <span>where <\/span>your therapist <span>will <\/span><span>begin the <\/span>assess<span>ment<\/span> <span>of <\/span>your current\u00a0problem<span>. You<\/span><span>r therapist <\/span><span>will <\/span><span>ask about<\/span><span> your history, injury, pain, and <\/span>functional goals for treatment.<span> Together you will<\/span> set goals, agree <span>on <\/span>a plan of care, and potentially start treatment<span>.<\/span><\/p>\n<p><span>Your treatment plan<\/span><span> will be determined <\/span><span>based on several factors including your examination\u00a0findings, current evidence, past\u00a0experience with treatment, and your doctor\u2019s prescription<\/span><span>.<\/span><\/p>\n<h4><strong><span>Duration of Visit<\/span><\/strong><\/h4>\n<p><span>Your evaluation<\/span><span> <\/span><span>will take <\/span><span><\/span><span>30-60 minutes. <\/span><span>If you do not have the chance to complete intake paperwork prior to your visit, please arrive 1<\/span>5<span> minutes early <\/span><span>to do so<\/span><span>. <\/span><\/p>\n<h4><strong><span>Student Involvement in Your Evaluation <\/span><\/strong><\/h4>\n<p>You will be evaluated and treated by a licensed physical therapist 100% of the time<span>. As a part<\/span> of\u00a0<span>Boston University <\/span>Sargent College<span>,<\/span> our mission includes providing clinical\u00a0experiences for students in <span>BU&#8217;s<\/span> Doctor of Physical Therapy program. Your care is always\u00a0directly supervised or performed by a licensed physical therapist.
