Some Schools require an advisor\u2019s signature for any changes to your schedule; contact the <a title=\"School\/College Contact Information\" href=\"https:\/\/www.bu.edu\/reg\/registration\/contact-info\/\">advising office<\/a> of your School\/College for more information.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"\/reg\/files\/2020\/09\/Audit-Form-5.pdf\">Audit Form<\/a><a href=\"\/reg\/files\/2020\/09\/Audit-Form-3.pdf\"><\/a><a href=\"\/reg\/files\/2020\/09\/Audit-Form.pdf\" a=\"\"><\/a><\/h4>\n<p>Undergraduate students who wish to register for a course with audit status or change a course from credit to audit status must submit this form to the Office of the University Registrar or their School\/College.\u00a0 Graduate students should contact their School\/College regarding policies and procedures for auditing courses.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"\/reg\/files\/2020\/09\/Incomplete-1.pdf\">Incomplete Form<\/a><\/h4>\n<p>Undergraduate students who wish to be assigned an incomplete grade, \u2018I\u2019, for a course must submit this form to their School\/College.\u00a0 Graduate students should contact their School\/College regarding policies and procedures for incomplete coursework.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"\/reg\/files\/2020\/09\/Personal-Data-Form.pdf\">Personal Data Update Form<\/a><\/h4>\n<p>Students who wish to change their name on Boston University records must submit this form to the Office of the University Registrar along with documentation of a legal name change. \u00a0This form can also be used to update Directory Information Restriction preferences.<br \/>\n<strong>Note<\/strong>: Even after graduation, privacy preferences remain in effect. For\u00a0example: if a student has restricted the release of their degrees, honors, and awards, the Office of the University Registrar will not confirm degrees earned to prospective employers or any other party without the student\u2019s permission.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"\/reg\/files\/2022\/01\/Cross-Registration-Petition-1.pdf\">Cross Registration Petition<\/a><a href=\"\/reg\/files\/2015\/02\/Cross-Registration-Form.pdf\"><\/a><\/h4>\n<p><a title=\"Boston Area Consortia (Cross Registration)\" href=\"https:\/\/www.bu.edu\/reg\/registration\/consortia\/\">Cross registration<\/a> is a formal agreement between Boston University and several local colleges and universities whereby BU students can attend courses at other institutions, and students from other institutions can attend courses at BU.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"https:\/\/www.bu.edu\/reg\/graduation\/diploma-replacement-2\/\">Diploma Replacement Request Form<\/a><\/h4>\n<p>Boston University and Wheelock College (1888-2018) graduates who would like to request a duplicate diploma may order a replacement by submitting an <a href=\"https:\/\/www.bu.edu\/reg\/graduation\/diploma-replacement-2\/\">online diploma replacement request form<\/a>.<\/p>\n<h4 style=\"margin-bottom: 0px;\"><a href=\"\/reg\/files\/2017\/04\/LOA-v8.pdf\">Graduate Student Leave of Absence Form<\/a><\/h4>\n<p><span>Boston University graduate students interested in taking a leave of absence from their academic program must submit the Leave of Absence Form to their School or College for review.
