Infant Care Practices Survey

Dear parent:

We appreciate your participation in this important study.  The Survey will take about 15-20 minutes to complete. However, if you cannot complete the Survey at one time, you may stop and return later.  As a reminder, the Survey is confidential and there are no right or wrong answers to any of these questions.

The Infant Care Practices Survey you are about to start has several sections:

  • Section 1 asks about infant feeding.
  • Section 2 asks about vaccinations. 
  • Section 3 asks questions about where and how you put your baby to sleep.
  • Section 4 asks about the use of pacifiers.

Each of the above sections begins with questions about what you originally planned to do when you brought your baby home from the hospital.  Then, you will answer questions about what you are doing now as well as what you plan to do in the future.  At the end of each section we ask about what influences you when caring for your baby.

  • Section 5 asks about your sources of childcare information.

In the final sections, we would like for you to answer a few general questions.

  • Section 6 asks about your baby’s doctor.
  • Section 7 asks general questions about yourself.

At any time during the Survey, if you feel that you need to explain any of your answers, please feel free to do so in the comment section at the end of each page.
