There is one template for performance evaluations for non-represented administrative employees – exempt and non-exempt.

An employee who would like to add comments to their written performance evaluation form may submit these comments in writing to their supervisor.

Where and How to Upload a Completed Performance Evaluation Form

Please be sure to follow any internal department processes and conduct the meeting with the employee prior to submitting the Performance Evaluation to Human Resources.
