Club Sport Handbook 21-22
All players should be advised to read through the Club Sports Handbook. Remember, some polices may change throughout the year, so please check back to this page for initial guidance on managing your team.

My FitRec
This is for members of this specific Club Sports team to submit their team payments. If you are not currently affiliated with the club please DO NOT submit a dues payment until you have been instructed to do so be the team’s President

Paying for Goods and Services

Check Request Form
Complete and submit to your advisor. Must be accompanied by proper backup documentation (W-9, invoice, official league/tournament communication, referee payment form, etc).

W-9 Form
This form is to be completed by new vendors and submitted either directly to your advisor by the vendor, or along with your Purchase Order or Check Request Form.

Official/Referee Payment Form
To be completed by game officials and a team representative and submitted to your advisor along with the Check Request Form and a W-9.

Purchase Request Form
Complete and submit (along with invoice) to your advisor. Please remember, there is a 10-day deadline on purchase requests.

Club Sports Van Travel Policies
All Club Sports participants should read the policies for using and traveling in Club Sports vans.

Van Driver Signup Form
Add your name to the list via the link above to sign up to become a certified van driver. The entire process can be found here.

Van Travel Request Form
Complete and submit this form to clubvans@bu.edu. The Club Sports Office will make every effort to accommodate your request. However, please note that PERD van usage is in high demand and we may not be able to accommodate all requests.

Charter Bus Request
If your club requires a bus (generally +30 members traveling), please fill out this form then contact your club’s advisor saying you submitted a request. Boston University has approved transportation vendors. It is important to submit any bus request to your club’s advisor, so your club is adhering to University guidelines as well as receiving the discounted member rate.

Lucid Travel Link
This is a the link to Boston University’s Lucid Travel account. Watch this training video for a step-by-step process on how to book a hotel using Lucid Travel. If your league/association is mandating you book through a room block please contact your advisor prior to booking.

Travel Itinerary Template

Community Service Report Form
Fill out and submit this form when your team completes a community service event. Submit at least 2 photos (1 group picture) to your advisor via email.

Facility Request Form
Complete and submit this form to your advisor via email two weeks or more prior to hosting an event in which participants are nonmembers of the University.
