Boston HealthNet Community Partnerships

Boston HealthNet, a health center-controlled network is a partnership comprised of Boston Medical Center, Boston University Chobanian & Avedisian School of Medicine, and 13 community health centers located throughout Boston’s neighborhoods and surrounding communities.  

In addition to providing care at Boston Medical Center and its affiliated clinics, our family medicine providers also practice at 13 Boston HealthNet community health centers across the city. These partnerships expand access to local high-quality primary care while maintaining seamless connection to leading specialists and hospital services at Boston Medical Center. 

Boston HealthNet centers strengthen communities by providing comprehensive, culturally responsive care close to home, which ensures that patients can access the care they need, when and how they need it most.  

Boston Health Care for the Homeless Program

Boston Health Care for the Homeless Program provides comprehensive health care services to individuals and families experiencing homelessness across Greater Boston. 

Care is delivered through more than 35 locations, including shelters, Jean Yawkey Place, street-based outreach, and other community settings. Services include primary care, behavioral health, recovery services, and case management, with a focus on continuity, dignity, and access. 

By combining clinical care with outreach and supportive services, the program plays a central role in expanding access and improving health outcomes for people experiencing homelessness. 

Codman Square Health Center

Codman Square Health Center has been serving the people of Dorchester since 1979. As a community-based outpatient health care and multi-service center, Codman integrates medical care with programs that support the health of the whole person and the whole community.  

As an innovative leader in regional, national and international efforts to improve community health, Codman is home to 250 multi-lingual and multi-cultural expert clinicians, medical staff and employees, and serves more than 100,000 patients annually.  

This practice serves as the primary practice site for the following faculty:

Daniel Driscoll-Neponset Health Center (Harbor Health)

Daniel Driscoll-Neponset Health Center, part of Harbor Health, serves residents of Dorchester and surrounding communities with accessible, community-based care.  

The center provides adult and family medicine, pediatrics, women’s health, behavioral health, recovery services, HIV prevention and support services, optometry, nutrition services, and on-site pharmacy access. As part of the Harbor Health network, patients have coordinated access to additional services across multiple locations. With a focus on convenience, continuity and whole-family care, Daniel Driscoll-Neponset Health Center helps expand access to high-quality services close to home. 

The Dimock Center

The Dimock Center is a Roxbury-based health and human services organization known for its whole-person approach to care. It’s one of the nation’s oldest community health centers, founded in 1862 as the New England Hospital for Women and Children. 

In addition to primary and specialty care, Dimock offers integrated behavioral health care, addiction and recovery services, early education, child and family services, and wraparound supports such as a food pantry and clothing closet. By bringing health care and social services together on one campus, the center supports individual well-being and long-term family stability. 

Dorchester House Multi-Service Center (DotHouse Health)

Dorchester House provides comprehensive, affordable, and accessible primary care alongside a wide range of on-site community resources. The center’s approach recognizes the connection between individual health, family well-being, and community strength, striving to improve the quality of life for all its residents.  

In addition to connection to medical services, Dorchester House offers case management, pro-bono legal services, WIC support, a food pantry, a youth center, a swimming pool, and a seasonal farmer’s market, bringing clinical care and community resources together under one roof.

Geiger Gibson Community Health Center (Harbor Health)

Geiger Gibson Community Health Center, part of Harbor Health, serves residents of Dorchester and surrounding communities with accessible, community-based care. 

The center provides primary care, women’s health, dental care, behavioral health, pharmacy, lab services, and social support services in one location. As part of the Harbor Health network, patients also benefit from coordinated access to additional services across multiple sites. With a focus on convenient, whole-family care, Geiger Gibson plays an important role in expanding access to high-quality services in the neighborhoods it serves. 

Harvard Street Neighborhood Health Center

Harvard Street Neighborhood Health Center serves residents of Dorchester and surrounding communities with a focus on equitable, high-quality community-based care. 

The center offers primary care, dental care, behavioral health, OB/GYN, eye care, podiatry, pharmacy and cancer screening, along with community programs such as a food pantry, WIC, veterans services and a Neighborhood Trauma Team. Its approach emphasizes access, continuity, and care that reflects the needs of the populations it serves. Through its deep roots in the community, Harvard Street Neighborhood Health Center plays a key role in expanding access to quality care across Dorchester.

Manet Community Health Center

Manet Community Health Center serves patients across the South Shore and Southeastern Massachusetts, with multiple locations across Quincy, Hull, Taunton and Attleboro. 

The center provides preventive, primary, same-day, mental health and substance use care, along with community outreach and prevention services. Manet also operates a school-based health center within Taunton Public Schools, expanding access for students and families. With a focus on accessible, community-based care, Manet Community Health Center plays an important role in supporting healthier communities across the region.

Brian Kroener 

Mattapan Community Health Center

Mattapan Community Health Center serves individuals living in Mattapan and neighboring communities, including Dorchester, Roxbury, and Hyde Park, as well as adjacent Boston suburbs like Brockton, Randolph, and Stoughton. It is the only community health center located in Mattapan and Hyde Park.   

The center provides comprehensive, accessible, culturally responsive primary and preventative health services designed to fulfill the center’s mission: to improve the quality of life for residents of Mattapan and surrounding communities.

Neighborhealth East Boston

For more than five decades, Neighborhealth has delivered high-quality care to individuals and families in East Boston and surrounding communities, regardless of age, income, insurance status, language, culture, or social circumstances. 

The center offers comprehensive services alongside innovative community programs, including the East Boston Farmers Market, the Elder Service Plan to support aging seniors at home, Project SHINE for multidisciplinary HIV care, Let’s Get Movin’, a lifestyle-change program for children and teens, and CATCH, which offers special care for children with complex medical needs.

South Boston Community Health Center

South Boston Community Health Center delivers high-quality primary and preventative care through an integrated “one-stop-shopping” model that provides medical and wellness services at one health center. 

In addition to offering extensive medical services, SBCHC also provides community resources such as the Living Healthy Speaker series, the South Boston Farmers Market, and the Institute for a Healthier Community, which engages youth in health, arts, and leadership development.

Carolyn Arnold, Isabela Brout, Evelyne Chiakpo, Aram Kaligian, Mekkin Lynch, Heather Miselis, Cierra Virtue. 

Upham’s Corner Health Center

The Upham’s Corner Health Center provides low-cost, comprehensive, culturally accessible primary care and social services to the residents of Dorchester. Each patient is assigned a personal primary care physician who coordinates specialty referrals and inpatient care with comfort and convenience. 

The center maintains close communication and medical record integration with Boston Medical Center to ensure continuity across care settings. Upham’s Corner prioritizes patient well-being in a respectful and accessible environment. 

Whittier Street Health Center

Whittier Street Health Center serves residents of Roxbury, North Dorchester and surrounding Boston neighborhoods with a focus on advancing health equity, social justice and economic well-being. 

The center provides comprehensive services across two locations, including primary, walk-in and specialty care, along with oral health, eye care, behavioral health, and social supports. Its integrated, patient-centered model connects clinical care with community-based resources to address both immediate health needs and the conditions that shape long-term well-being.
