{"id":14232,"date":"2017-04-25T15:00:00","date_gmt":"2017-04-25T19:00:00","guid":{"rendered":"https:\/\/www.bu.edu\/ehs\/?page_id=14232"},"modified":"2018-12-11T15:24:29","modified_gmt":"2018-12-11T20:24:29","slug":"electrical-safety-lock-outtag-out-in-depth","status":"publish","type":"page","link":"https:\/\/www.bu.edu\/ehs\/residential-safety-home\/residential-safety-programs-services\/campus-and-clinical-safety\/electrical-safety\/electrical-safety-lock-outtag-out-in-depth\/","title":{"rendered":"Electrical Safety &#038; Lock Out\/Tag Out"},"content":{"rendered":"<h3>Basic Electrical Safety<\/h3>\n<ul>\n<li>Electrical devices must be properly grounded: 3-prong or double insulated.<\/li>\n<li>Electrical devices must be UL approved or similar approval.<\/li>\n<li>If working near water, use designated GFCI (ground fault circuit interrupted) outlets.<\/li>\n<li>Remove any damaged device from service immediately.<\/li>\n<li>Never use \u201ccheaters\u201d (convert 3-prong to 2-prong plugs).<\/li>\n<li>Disconnect any electrical\/medical device that:\n<ul>\n<li>Throws sparks<\/li>\n<li>Blows a fuse<\/li>\n<li>Gives even the slightest shock<\/li>\n<\/ul>\n<\/li>\n<li>Notify your supervisor and others in the immediate area.<\/li>\n<li><strong>Call the Control Center: MED: 617 -638-4144(MED)\/CRC: 617-353-2105<\/strong>\u00a0or your vendor for repairs.<\/li>\n<li>Check cords\/devices for frayed or bare wires, or other defects prior to use.<\/li>\n<li>Avoid using extension cords.<\/li>\n<li>Follow the manufacturer\u2019s instructions for use and maintenance.\n<ul>\n<li>BMC (Hospital) staff \u2013 reference <a href=\"http:\/\/internal.bmc.org\/clinicalengineering\/\">Clinical Engineering\u2019s web site<\/a><\/li>\n<li>Hospital Patient Equipment \u2013 contact <strong>Clinical Engineering at 617 -638-6061 or 617 414-4692<\/strong>.<\/li>\n<\/ul>\n<\/li>\n<li>Never operate clinical equipment without proper training.<\/li>\n<li>Attach tag\/note: \u201cDo Not Use\u201d, or a Broken Equipment Tag for BMC patient care equipment.<\/li>\n<li>Incidents involving patient equipment must be reported through the STARS Incident Reporting System on the BMC Intranet.<\/li>\n<\/ul>\n<h3>Lock Out\/Tag Out<\/h3>\n<p>The Hazardous Energy Control Program establishes the minimum requirements for the Lockout and Tagout (LOTO) of energy-isolating devices to prevent an undesirable release of hazardous energy during any servicing, maintenance, or modification activity. LOTO will be used to ensure that the affected equipment or system is isolated from all potential hazardous energy and is locked\/tagged out before authorized employees perform any maintenance activities where the unexpected startup or release of stored energy could cause a serious personal injury or a fatality.<\/p>\n<p>The\u00a0written Energy Control Program\u00a0provides personnel with procedures for isolating equipment during servicing or maintenance. Personnel are trained in accordance with their classification under the program. Periodic reviews of the written LOTO procedures are performed to identify any changes that must be made to the procedures and to verify personnel are correctly performing LOTO procedures.<\/p>\n<p>The Occupational Safety and Health Administration has established employee classifications within the control of hazardous energy (Lockout\/Tagout) standard, 29 CFR 1910.147. Depending on their role, personnel are provided training so they can perform their duties under the\u00a0Program. The classifications of employees are Authorized, Affected, and other.<\/p>\n<p><strong>Affected Employee<\/strong><br \/>\nAn employee whose job requires him\/her to operate or use a machine or equipment on which servicing or maintenance is being performed under lockout or tagout, or whose job requires him\/her to work in an area in which such servicing or maintenance is being performed.<\/p>\n<p><strong>Authorized Employee<\/strong><br \/>\nAn &#8216;authorized employee&#8217; is defined as a person who locks out or tags out machines or equipment in order to perform servicing or maintenance on that machine or equipment. An affected employee becomes an authorized employee when that employee&#8217;s duties include performing servicing or maintenance covered under this section.<\/p>\n<p><strong>Departmental Procedures<\/strong><br \/>\nEnvironmental Health and Safety works closely with affected departments to develop equipment specific procedures. EHS and those departments maintains those procedures for departmental use.<\/p>\n<p><strong>Other Employee<\/strong><br \/>\nOther employees are those whose work operations are or may be in an area where energy control procedures may be utilized.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Basic Electrical Safety Electrical devices must be properly grounded: 3-prong or double insulated.
