These requirements include, but are not limited to, receipt of a satisfactory criminal background history report, and submission of official degree-conferring dental transcript.<\/h2>\n<h3><a href=\"https:\/\/www.bu.edu\/dental\/files\/2020\/12\/2021-DMD-AS-Acceptance-Confirmation-w-Enrollment-and-Deposit-Instructions.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Acceptance Confirmation and Deposit\u00a0<\/strong><\/a><\/h3>\n<p>To hold a space in the incoming class, DMD Advanced Standing applicants who are offered admission are required to submit an Acceptance Confirmation form (a signed statement of intention to enroll) and a $3,000 nonrefundable tuition deposit. The nonrefundable tuition deposit is applied to the student account toward the first semester of enrollment.<\/p>\n<h3><strong>Official Qualifications Documentation<\/strong><\/h3>\n<p>Qualifying test and academic reports submitted for your CAAPID application directly from the testing agency or credential evaluator will be included in the entering student record and considered official.\u00a0 If documentation in the CAAPID application is incomplete or does not qualify, the entering student must submit official reports to Admissions\u00a0directly from the testing agency or credential evaluator prior to matriculation.<\/p>\n<ul>\n<li><strong>Test report from the Joint Commission on National Dental Examination <\/strong><\/li>\n<li><strong>TOEFL<span>\u00a0 <\/span><\/strong>GSDM Admissions: TOEFL Institution code 3081, TOEFL Department code\u00a038<\/li>\n<li><strong>ECE course-by-course transcript evaluation<\/strong><\/li>\n<\/ul>\n<h3><strong>Transcripts and Diplomas<\/strong><\/h3>\n<p>Accepted DMD Advanced Standing applicants must submit an original, official dental school transcript, indicating the date the dental degree was earned.\u00a0 Accepted applicants who hold additional degrees should submit an original, official transcript and a scan of the diploma for each degree earned.\u00a0 Original, official transcripts must be sent directly from the educational institution to Boston University Henry M. Goldman School of Dental Medicine DMD Advanced Standing Admissions.\u00a0\u00a0An enrolling applicant may submit an official transcript sent to the applicant\u2019s address only if the transcript remains unopened and sealed in its original envelope.\u00a0 Documents that have been handled by the applicant are considered unofficial.<\/p>\n<p>Original documents are required; if the report is issued electronically directly from the credential originator to GSDM Admissions, the report is considered original documentation.<span>\u00a0 <\/span>We cannot accept scans or photocopies of required official documentation. Any materials submitted become part of the applicant\u2019s record and will not be returned.<\/p>\n<h3><strong>Criminal Background Check<\/strong><\/h3>\n<p>Boston University GSDM participates in the ADEA partnership with Certiphi Screening for criminal background check (CBC) processing for accepted applicants. Admitted applicants receive an email from Certiphi Screening with instructions on how to complete the background check process. Satisfactory background check results are required for matriculation into the program.<\/p>\n<h3><strong>BU Computing Account<\/strong><\/h3>\n<p>Entering students will be given instructions on creating a Boston University computing account. The Boston University account and Boston University email are required for a variety of important processes and should be set up as soon as available.<\/p>\n<h3><a href=\"http:\/\/www.bu.edu\/shs\/resources\/ihr\/mchealthform\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Incoming Health Requirements<\/strong><\/a><\/h3>\n<p>All enrolling students submit mandatory health and immunization information to\u00a0<a href=\"http:\/\/www.bu.edu\/shs\/\" target=\"_blank\" rel=\"noopener noreferrer\">Student Health Services<\/a>. All dental students must be in clinical compliance status as determined by Boston University Henry M. Goldman School of Dental Medicine, Boston University Student Health Services, and state and local requirements.<\/p>\n<p>The Commonwealth of Massachusetts requires all health care students to be immunized against hepatitis B or provide serologic proof of immunity. Hepatitis B immunization requires a three-dose regimen of vaccine, which must be completed prior to matriculation. Evidence of inoculation (all three doses) must be provided at matriculation.<\/p>\n<h3><a href=\"http:\/\/www.bu.edu\/isso\/getting-started\/documents-procedures\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>International Student Documentation<\/strong><\/a><\/h3>\n<p>Following enrollment confirmation, accepted foreign nationals submit personal and financial documentation to Admissions when requested, via the Applicant Gateway Collect Document activity. Admissions will review the documents and forward them to the Boston University International Students\u00a0&amp; Scholars Office (ISSO).\u00a0 ISSO\u00a0then produces the appropriate document to enable entering international students to apply for a student visa, if required.\u00a0 Canadian students must obtain the immigration document from the ISSO but do not require a visa to study in the USA.<\/p>\n<h3><a href=\"https:\/\/www.bu.edu\/dental\/students\/matriculation\/pre-doctoral\/materials\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Program Start<\/strong><\/a><\/h3>\n<p>Boston University Henry M. Goldman School of Dental Medicine holds\u00a0<a href=\"https:\/\/www.bu.edu\/dental\/students\/matriculation\/pre-doctoral\/\">mandatory orientation sessions<\/a>\u00a0as well as academic sessions\u00a0during the first week of the program.
