DMD\/ DMD AS<\/strong><\/h2>\n

You will be eligible to sign out of your locker AFTER<\/strong> all clinical competencies in SPS have been signed out by the appropriate faculty member.<\/span>\u00a0<\/span><\/p>\n

When you are ready to sign out of your locker, please complete the following:<\/span>\u00a0<\/span><\/p>\n

    \n
  1. Verify in SPS that you have been signed out of all clinicals<\/span><\/span><\/li>\n
  2. Visit the <\/span>GSDM Portal<\/span><\/a> and first enter your \u201cPostgrad Career Form\u201d via the Student Affairs tab\u00a0<\/span><\/span>\n
      \n
    1. This form collects information about your post-graduation plans for Career Resources<\/span><\/span><\/li>\n
    2. If you do not see \u201cPostgrad Career\u201d listed as a drop down, please use Safari, Internet Explorer, or Firefox (Google Chrome may not display the form)<\/span><\/span><\/li>\n<\/ol>\n<\/li>\n
    3. Clean out your locker and take two photos –\u00a0<\/span><\/span>\n
        \n
      1. photo of the front of the locker with the locker number clearly visible<\/span><\/span><\/li>\n
      2. Photo of the inside of the clean locker<\/span><\/span><\/li>\n<\/ol>\n<\/li>\n
      3. <\/span>Send email, with both photos attached, to <\/span>gsdmsa@bu.edu<\/span><\/a> and cc: <\/span>toledano@bu.edu<\/span><\/a>, informing us that you have completed steps 1 through 3<\/span>. <\/span>\u00a0If someone else is using your locker, please send us a photo of the illegally occupied locker.<\/span><\/li>\n<\/ol>\n

        Student Affairs will review all relevant information and if the locker sign-out criteria is met, we will sign you out remotely, within 1-2 business days.\u00a0 You will receive an email confirmation when your locker sign-out has been completed.<\/span>\u00a0<\/span><\/p>\n
