This form is for current students who wish to switch either their current degree program to a different degree program <span>or<\/span> their current certificate program to a different certificate program. You may not switch from one program type to another with this form.<br \/>\n<\/span><\/p>\n<p><span class=\"gform_description\">Upon receipt of this form, the Department will review your admissions documents to determine if any additional prerequisites are required for the new program and will audit your current transcript to determine which courses may apply to the new program. Please be aware that different programs may have additional prerequisites that will need to be taken for full admission. Further, not all courses may count from one program to the next.
