How To Upload Your Vaccine\/Documentation<\/strong><\/p>\n

  1. Log in to the Occupational Health Connect Portal<\/a>\u00a0<\/span>using your BU username and password via single sign-on.<\/li>\n
  2. Select the \u201cUpload Documents\u201d option from the left sidebar menu.<\/strong>\"\"<\/li>\n
  3. Indicate the type of document you are uploading from the available dropdown selections.\"\"<\/li>\n
  4. Answer the following questions after making your selection. The document being uploaded must contain<\/span>\u00a0<\/span>your <\/span>Full Legal Name<\/span> <\/strong><\/span>AND<\/strong><\/span> <\/strong><\/span>Date of Birth.<\/span>\"\"<\/li>\n
  5. Click to choose y<\/span>our file, ensuring it is either in PNG, PDF, or JPG file format only.<\/li>\n
  6. To complete your document upload, select the green\u00a0<\/span>SUBMIT<\/span>\u00a0<\/span>button.<\/li>\n
  7. After you select SUBMIT:\n
      \n
    • You will see a brief Thank You message.<\/li>\n
    • You will be brought back to the Home screen.<\/li>\n
    • You will receive an email from\u00a0<\/span>ohcinfo@bu.edu<\/strong>\u00a0<\/span>within 5 minutes of submission to confirm your upload is complete.<\/li>\n
    • An additional email will arrive when your document has been reviewed and accepted by clinical staff.\n

      • If you need to upload multiple documents (e.g. test results and a provider note), complete one submission process for each document.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n
