2008 REGISTRATION INFORMATION

We are now accepting registration for our 2008 Racing Camp!

In order to register for the racing camp, you need to fill out a REGISTRATION FORM.

Please fill out this form, include your $250.00 deposit check, and make it out to:

Campers will NOT be registered until $250.00 deposit is received.

In addition to the registration form, there are seven other critical forms we need before JULY 1, 2008

PARENTAL RELEASE FORM
This is a general waiver, but also needed is health insurance information, including a copy of your health insurance card.

HEALTH FORM
This form needs to be filled out by a physician.

MEDICATION AUTHORIZATION FORM
If your daughter takes any kind of prescription medication, then this form needs to be completed. This gives the camp permission to administer the medication if necessary.

DORM RULES
For overnight campers. Please read and review with your daughter and have her sign.

TRAVEL PLANS
For overnight campers

WALKING IN BOSTON PERMISSION FORM
This form gives your daughter permission to leave the dorm for short periods of time to go around the corner to a convenience store or coffee shop.

DEWOLFE BOAT CLUB MEMBERSHIP FORM
This form makes each camper a member of DeWolfe Boat Club and therefore covered by USRowing liability insurance.

USRowing RELEASE OF LIABILITY FORM

If these forms are not received in their entirety before JULY 1st, 2008, then your place at the camp will be forfit and your deposit will be returned less a $25.00 processing fee.
