What does this mean?

I get the no copayment. But a 15 copayment is nothing. What about contact lens evaluation? That's usually $100. Also what does 40% discount on glasses mean? Most all eyeglass places say they offer "discounts". But I always end up paying $250 or more for a pair. So, what does this really mean?
