Proctored Exam Schedule 2014-2015

For STM and PhD students: This document indicates the schedule of proctored exam dates. Students interested in signing up for a date must do so at least two weeks prior to the exam date. Contact Robyn Kinch to sign up or for more details at 353-3045. Space is limited. Students may also sit for exams with their advisor or a proctor of their advisor’s choice. For specific instructions regarding examinations, it is critical that students consult their degree program handbook carefully.

General Petition Form

For STM, PhD, and DMin students: This form can be used for any petition to the Advanced Studies Committee. All petitions must be approved by the student’s advisor(s), and course instructor where applicable, before they can be reviewed by the Advanced Studies Committee. If your petition involves a directed study, a minimum one-page synopsis of course requirements or syllabus will be required in addition to the petition form.

Dissertation Prospectus Approval Form

For PhD students: Each advisor must approve of a prospectus before it can be forwarded to the Advanced Studies Committee for review. Advisors can indicate their approval using this form. Once submitted to the ASC, the prospectus becomes a public document within the school. The student must sign (physical signature on this form or electronically) stating that they have read and understood this.

Project Thesis Approval Form

For DMin students: Both the Project Director and Associate Project Director must approve of the project thesis proposal before it can be forwarded to the ASC for review. Advisors can indicate their approval using this form. Once submitted to the ASC, the project thesis proposal becomes a public document within the school. The student must sign (physical signature on this form or electronically) stating that they have read and understood this.
