Entering Student Requirements

Welcome to Boston University!

All entering Boston University students have pre-matriculation health requirements that must be completed prior to your first term at Boston University. The individual class links below will provide you with specific instructions for your class requirements.

IMPORTANT! Failure to complete the required health requirements will result in a registration hold on your Student Account. This means you will not be able to register for classes.

Note:   These instructions are for Medical (MD), Dental, or Clinical Graduate Medical Sciences Program students.  If you are not enrolled in one of these programs, CLICK HERE to access the correct instructions.

Please follow these three steps to complete the Immunization and Physical form process:

1. Download the required Immunization & Physical Form and have them completed by a health care provider.

2. Once completed and signed, please follow these steps to upload your form to our secure portal Patient Connect.

A) Log into Patient Connect using your Kerberos account issued by the university. If you have a problem with your Kerberos account, please see FAQ’s at the bottom of the page.

B) Click the “Forms” option on the left menu bar.

C) Click “Immunization & Physical Form Upload.”

D) Follow the instructions to upload your “Immunization & Physical Form” and click Save. 

3. Using the completed “Immunization & Physical Form” from step 1, please enter your immunization information into the Immunization Portal by following the steps listed below.

A) Log into the Immunization Portal using your Kerberos Account issued by the university. If you have a problem with your Kerberos account, please see FAQ’s at the bottom of the page.

B)  Enter your immunization record online using the completed “Immunization & Physical Form” from step 1.  If you have questions, you can always save or cancel your online record and log back in at a later time.

C) Review and click Submit when you are satisfied with the online record.

D) Once your record has been submitted, you will no longer be able to access the Immunization Portal.  If there are issues with the online record, an SHS team member will contact you directly to resolve the issue.

We encourage you to upload and enter your immunization history into our portals because it will update your compliance status much faster than standard mail. However, if you cannot complete any of the steps above because of technological restrictions, please send all completed paper forms to:

We cannot accept health information received via email

1. Complete the required online “Incoming Health History Form” accessed through Patient Connect.  IMPORTANT: Accessing the health form online requires a Kerberos Username and Password issued from the University.    If you have an issue with your Kerberos account, please see FAQ’s at the bottom of the page.

Please follow these steps to submit your health history information:

A) Log into Patient Connect using your Kerberos Account issued by the university.

B) Click the “Forms” option on the left menu bar.

C) Click the link for the “Incoming Health History Form” at the bottom of the page.

D) Enter, review, and submit your health history information.

1. STUDENTS WHO WILL BE UNDER AGE 18 ON SEPTEMBER 1 of the current year: Download and complete the Consents & Acknowledgments Form. Once completed and signed, log in through Patient Connect to upload a copy of the Consents & Acknowledgments Form to our portal. If you cannot upload, please mail a copy of your completed form to SHS. 

IMPORTANT: Accessing Patient Connect requires a Kerberos Username and Password issued from the University.  If you have an issue with your Kerberos account, please see FAQ’s at the bottom of the page.

Please follow these steps to submit your Consents & Acknowledgments Form:

A) Log into Patient Connect using your Kerberos Account issued by the university.

B) Click the “Forms” option on the left menu bar.

C) Click the link for the “Immunization & Physical Form 2016 Upload” at the bottom of the page. IMPORTANT: You will upload your consent form to the same place that your immunization records are uploaded.

D) Click “Add Immunization & Physical Form 2016″ to upload your file in one of the following formats: PNG, JPG, JPEG, or GIF. At this time, we cannot accept any file uploaded in PDF format or anything else.

E) Click Save. Once saved, click Exit to proceed to the next step.

1. All incoming Student-Athletes (varsity & club) MUST also complete an “Athletic Screening Questionnaire.” Please follow these steps to complete the Athletic Screening Questionnaire process:

A) Log into Patient Connect using your Kerberos Account issued by the university.

B) Click the “Forms” option on the the left menu bar.

C) Click the link for the “Athletic History Screening Questionnaire” online form at the bottom of the page.

D) Enter, review, and submit your questionnaire.

2. If you are under 18, please complete the Release For Minors Waiver Form for a Parent/Legal Guardian signature and follow the instructions to submit this form to Athletic Training Services.

A) Complete the Release For Minors Waiver Form and scan it to your mobile device or computer.

B) Log into Patient Connect using your Kerberos Account issued by the university.

C) Click “Messages”, “New Message”, and select “I want to communicate with the Athletic Training Department.”

D) Select the option to “Submit Release For Minors Waiver Form.”

E) Upload the completed Release For Minors Waiver Form as an attachment and click “Send.”

If you have questions or require support, you can send a secure message by logging into Patient Connect, select “Messages”, “New Message”, and follow options to send Athletic Training a “General Message.”

YES! We require all incoming students to complete this process to comply with Boston University admissions standards.

YES! You must complete both the Immunization & Physical Form AND health history information regardless of whether or not you are accepting the Boston University Insurance plan.

A Kerberos account is a unique and secure account used to access a variety of online services throughout your tenure here at Boston University.

Instructions on how to obtain a Username and Kerberos Password were already provided to you by the Admissions Office for Undergraduate students and from the individual Graduate Schools for Graduate School students. If you have not received these instructions, please contact the Admissions Office (Undergraduate students) or your Graduate School (Graduate School students). If you don’t remember your password, CLICK HERE and follow the instructions issued by the BU IT Help Center.

University systems may take time to update if you are a new student. Especially if your Kerberos account was recently created. In which case, you should wait a couple of days and then try again. We are always here to help, so please feel free contact SHS Help for support and be sure to include your BU-ID and the details of your issue. We will review your request within 48 hours and send you an email update.

YES! A doctor from a foreign country or a doctor in the US can complete and sign the Immunization & Physical form.
