Entering Student Requirements

Due: The fall semester is due by August 1 and the spring semester is due by January 14.

Welcome to Boston University!

All entering Boston University students have pre-matriculation health requirements that must be completed prior to your first term at Boston University. The individual class links below will provide you with specific instructions for your class requirements.

IMPORTANT! Failure to complete the required health requirements will result in a registration hold on your Student Account. This means you will not be able to register for classes.

Instructions

Note:   These instructions are for Medical (MD), Dental, or Clinical Graduate Medical Sciences Program students.  If you are not enrolled in one of these programs, CLICK HERE to access the correct instructions.

Section 1: Complete required Immunization & Physical Form

1. Download the required Immunization and Physical Form and have them completed by a health care provider. IMPORTANT: Please keep a copy of the completed paper Immunization & Physical Form. You will need to mail or fax them to Student Health Services at the end of this process.

2. Using the completed “Immunization & Physical Form” from step 1, please enter your immunization information into the Immunization Portal by following the steps listed below. IMPORTANT: Accessing the Immunization Portal requires a Kerberos username and password issued by the University.   If you have an issue with your Kerberos account, please see FAQ’s at the bottom of the page.

Follow these steps to input your immunization history online:

A) Log into the Immunization Portal using your Kerberos Account issued by the university.

B)  Enter your immunization record online using the completed “Immunization & Physical Form” from step 1.  If you have questions, you can always save or cancel your online record and log back in at a later time.

C) Review and click Submit when you are satisfied with the online record.

D) Once your record has been submitted, you will no longer be able to access the Immunization Portal.  If there are issues with the online record, an SHS team member will contact you directly to resolve the issue.

Section 2: Enter your required health history

1. Complete the required online “Incoming Health History Form” accessed through Patient Connect.  IMPORTANT: Accessing the health form online requires a Kerberos Username and Password issued from the University.    If you have an issue with your Kerberos account, please see FAQ’s at the bottom of the page.

Please follow these steps to submit your health history information:

A) Log into Patient Connect using your Kerberos Account issued by the university.

B) Click the “Forms” option on the left menu bar.

C) Click the link for the ”Incoming Health History Form” at the bottom of the page.

D) Enter, review, and submit your health history information.

Section 3: Complete Consents & Acknowledgements Form (If Under 18 Years Old)

1. STUDENTS WHO WILL BE UNDER AGE 18 ON SEPTEMBER 1 of the current year: Download and complete the Consents & Acknowledgments Form. IMPORTANT: Please keep a copy of the completed form. You will need to mail or fax it to Student Health Services at the end of this process.

Section 4: Complete Athletic Screening Questionnaire (Varsity or Club Athletes Only)

1.  ALL incoming Student-Athletes (varsity & club) MUST also complete an “Athletic Screening Questionnaire.”  Please follow these steps to complete the Athletic Screening Questionnaire process:

A) Log into Patient Connect using your Kerberos Account issued by the university.

B) Click the “Forms” option on the the left menu bar.

C) Click the link for the ”Athletic History Screening Questionnaire” online form at the bottom of the page. If the form is not present, please contact Athletic Training Services athtrn@bu.edu or 617.353.2746.

D) Enter, review, and submit your questionnaire.

Section 5: Mail or Fax Completed Paper Forms

Now that you have completed and reviewed all of the steps above, the last step is to mail all completed paperwork to Student Health Services for review.  Failure to mail completed forms will result in a  hold on your Student Account which means you may not be able to register for classes.

Frequently Asked Questions

Do I have to complete both the Immunization & Physical Form AND health history information?

YES! We require all incoming students to complete this process to comply with Boston University admissions standards.

Do I need to complete both the Immunization & Physical Form AND health history information if I'm waiving out of the Boston University Insurance Plan?

YES! You must complete both the Immunization & Physical Form AND health history information regardless of whether or not you are accepting the Boston University Insurance plan.

What is a Kerberos Account?

A Kerberos account is a unique and secure account used to access a variety of online services throughout your tenure here at Boston University.

I haven’t received instructions on how to obtain a KERBEROS password. What should I do?

Instructions on how to obtain a Username and Kerberos Password were already provided to you by the Admissions Office for Undergraduate students and from the individual Graduate Schools for Graduate School students. If you have not received these instructions, please contact the Admissions Office (Undergraduate students) or your Graduate School (Graduate School students). If you don’t remember your password, CLICK HERE and follow the instructions issued by the BU IT Help Center.

I have already created my Kerberos account but can't access Patient Connect

If you have already created a Kerberos account but can’t Log into to Patient Connect, please contact SHS Help for support. Please include your UID and the details of your issue.

Is there a link to Boston University’s Health Insurance requirements?

YES! These requirements are separate from the health/immunization requirements. Click HERE for more information.

Is a signature from a physician outside the United States acceptable on the paper Immunization & Physical Form?

YES! A doctor from a foreign country or a doctor in the US can complete and sign the Immunization & Physical form.
