2012 Entering Full Time Student Requirements

Welcome to Boston University!
All entering Boston University students have pre-matriculation health requirements that must be completed prior to your first term at Boston University. The individual class links below will provide you with specific instructions for your class requirements.
IMPORTANT! Failure to complete the required health requirements will result in a check-in hold on your Student Account. This means you may not be able to check-in for classes for the Fall 2012 term!
Health Form Instructions
Note: These instructions are for non-medical full time students taking undergraduate or graduate classes on the Charles River Campus, including the School of Public Health (SPH) . If you are taking classes in the School of Dental Medicine (SDM) or Medical School (MED), CLICK HERE to access the correct instructions.
Section 1: Complete Required Online Health History and Immunization Forms
1. ALL STUDENTS: Download the required BU Charles River Campus Immunization and Physical Form 2012and have them completed by a health care provider. **Please keep a copy of the completed paper Immunization Forms. You will need to mail or fax them to Student Health Services at the end of this process.
This includes:
A) Page 1 – Meningitis Waiver at bottom of page 1 is only to be filled out if you are waiving the meningitis shot requirement.
B) Page 2 – Immunization information REQUIRED of ALL full time students (completed by a health care provider).
C) Page 3 – Physical Information and Optional Immunizations for all full time Undergraduate and Graduate students on the Charles River Campus.(completed by health care provider).
D) Page 4 – Required of all students who answer YES to any question listed on the paper form. (completed by a health care provider) *IMPORTANT: Tuberculosis testing done prior to September 1, 2011 will NOT be accepted!*
2. STUDENTS WHO WILL BE UNDER AGE 18 ON SEPTEMBER 1, 2012: Download and complete the Consents and Acknowledgments Form. **Please keep a copy of the completed form. You will need to mail or fax it to Student Health Services at the end of this process.
3. Complete the required online “Incoming Health History Form 2012″ accessed through Patient Connect. IMPORTANT: Accessing the health form online requires a Kerberos Username and Password issued from the University. See FAQ’s at the bottom of the page for more information. Once you have a Username and a Kerberos Password you have access to Patient Connect.
Please follow these steps to complete the health history part of the Health Form process:
A) Logon to Patient Connect using your Kerberos Account issued by the university.
B) Click the “Forms” option on the left menu bar.
C) Click the link for the ”Incoming Health History Form 2012″ at the bottom of the page.
D) Enter, Review, and Submit your health history information. Verify the form is no longer accessible on the Patient Connect forms page.
Are you an incoming student athlete? If so, please complete step 4 below.
If not, please skip step 4 and proceed to Section 2.
4. ALL incoming Student-Athletes (varsity & club) MUST also complete an Athletic Screening Questionnaire in addition to the Incoming Health History Form. Please Note: Any questions or concerns, please contact Athletic Training Services athtrn@bu.edu or 617.353.2746
Please follow these steps to complete the Athletic Screening Questionnaire process:
A) Logon to Patient Connect using your Kerberos Account issued by the university.
B) Click the “Forms” option on the the left menu bar.
C) Click the link for the ”Athletic History Screening Questionnaire 2012″ online form at the bottom of the page.
D) Enter, Review, and Submit your questionnaire. Verify the form is no longer accessible on the Patient Connect forms page.

Section 2: Mail or Fax Completed Paper Forms
Now that you have completed and reviewed all of the steps above, the last step is to mail all completed paperwork to Student Health Services for review.

Frequently Asked Questions
Do I have to complete both the Health History Form AND the paper Boston University Immunization Record?
YES! We require a completed online copy of the Health History Form and a hardcopy of the Boston University Immunization record to authenticate that the student is compliant with Boston University admissions standards.
If I'm waiving out of the Boston University Insurance Plan do I still need to complete the online Health History Form and submit my paper Immunization Record?
YES! You must complete the online Health History Form and submit your paper Immunization Record regardless of whether or not you are accepting the Boston University Insurance plan.
What is a Kerberos Account?
A Kerberos account is a unique and secure account used to access a variety of online services throughout your tenure here at Boston University.
I haven’t received instructions on how to obtain a KERBEROS password. What should I do?
Instructions on how to obtain a Username and Kerberos Password were already provided to you by the Admissions Office for Undergraduate students and from the individual Graduate Schools for Graduate School students. If you have not received these instructions, please contact the Admissions Office (Undergraduate students) or your Graduate School (Graduate School students). If you don’t remember your password, CLICK HERE and follow the instructions issued by the BU IT Help Center.
I have already created my Kerberos account but can't access Patient Connect

Is there a link to Boston University’s Health Insurance requirements?
YES! These requirements are separate from the health/immunization requirements. Click HERE for more information.
Is a signature from a physician outside the United States acceptable on the paper Immunization & Physical Form?
YES! A doctor from a foreign country or a doctor in the US can complete and sign the Immunization & Physical form.
