Student Worker Accident Procedures

If a student employee suffers an accident in the course of performing his/her job duties, a supervisor or director of the student employee should complete a Commonwealth of Massachusetts Accident Report Form 118 or Form 101 and a Boston University Accident Report and Analysis Form OH-1.

Form 118 should be filled out when less than 5 calendar days or NO loss of work time is involved.

Form 101 should be filled out if there are more than 5 calendar days of work lost as a result of an accident.

Form OH-1 should always be filled out and submitted along with form 118 or Form 101.

IMPORTANT: The student employee who suffers an industrial accident should not fill out the accident form. The accident report should be filled out by the student's supervisor within 24 hours of the accident. Any delay in filing may cause the University to be fined, as well as possibly jeopardizing an employee's right to receive benefits.

You may download the Form 118, Form 101 and Form OH-1 from the Office of Risk Management web site. This web site also provides more detailed information regarding Workers' Compensation and Accident Reporting procedures.

Once completed, Form 118 or Form 101 and Form OH-1 should be routed to:

Any medical bills should also be forwarded to Mary Ann French. This paperwork will be reviewed and forwarded to the Office of Personnel for processing.

The Office of Risk Management should be contacted immediately at (617) 353-3020 when a student employee is severely injured on the job. If an accident occurs after normal office hours (Risk Management office hours are Monday through Friday, 9-5), or on weekends, the office should be apprised of the accident on the next business day.

If it is necessary for a student employee to be taken to the hospital, suitable transportation should be provided and the student should be accompanied to the hospital. Transportation arrangements back to the student’s residence should also be provided.
